甲状旁腺癌的外科诊治

目的探讨甲状旁腺癌（parathyroid carcinoma，PTC）的诊断与外科治疗方法。方法对郑州大学第一附属医院1997年至2010年收治的6例PTC患者的临床表现、实验室检查及手术治疗进行回顾性分析。结果6例患者血钙及血清甲状旁腺激素（PTH）均高于正常值，术后均发生短时的低血钙症。均行甲状旁腺癌切除术加患侧甲状腺近全切除术。5例术中冷冻明确诊断，1例术后石蜡病理及免疫组织化学确诊。结论术前结合血钙、PTH与超声、CT、99Tcm-MIBI影像学检查有助于定性、定位诊断，手术切除甲状旁腺癌以及同侧甲状腺是一种良好的手术方法。

Diagnosis and surgical treatment of parathyroid carcinoma

Objective To evaluate the diagnosis and surgical treatment of parathyroid carcinoma （FFC）. Methods Six patients with parathyroid carcinoma who were treated in our hospital from 1997 to 2010 were analysed retrospectively. Results Patients showed hypercalcemia and elevated parathyroid hormone （PTH）. All patients presented with temporary hypocalcemia after surgery. Parathyroidectomy and ipsilateral neartotal thyroidectomy was performed in all patients. Frozen section established diagnosis in 5 patients. Routine histopathology combined with immuohistochemistry established the diagnosis in one patient. Conclusions Blood calcium, blood FI＇H, ultrasonography, CT and 99 Tcm _ MIBI are helpful to diagnosis. The operative removal of parathyroid carcinoma with thyroid at the same side is a better radical treatment.
