腹腔镜辅助阴式子宫全切术与经腹子宫全切术的临床研究

目的探讨腹腔镜辅助阴式子宫全切术（LAVH）与经腹子宫全切术（TAH）的临床效果比较。方法选择2008年5月至2009年6月期间患有子宫良性病变在本院行子宫全切术的患者96例，其中行腹腔镜辅助阴式子宫全切术（LAVH组）48例，行经腹子宫全切术（TAH组）48例，比较两组患者术中及术后的情况，术后随访1年，比较术后的生活质量及性生活情况。结果LAVH组术中出血量、住院时间、肛门排气时间及术后并发症均低于TAH组（P〈0．05）。LAVH组生活质量及性生活均优于TAH组。结论与经腹子宫全切术相比，腹腔镜辅助阴式子宫全切对患者损伤小、恢复快，节约住院时间，具有广泛的临床应用前景。

Clinical study of laparoscopic assisted vaginal hysterectomy and trans abdominal hysterectomy

Objective To study the clinical effects of laparoscopic assisted vaginal hysterectomy （LAVH） and abdominal hysterectomy （TAH）. Methods Choose 92 patients with benign uterine trea- ted by hysterectomy from May 2008 to June 2009, including 48 cases of laparoscopic assisted vaginal hys- terectomy （ LAVH group） and 48 cases of abdominal hysterectomy （TAH group）. Two groups were compared the situation during and after surgery, patients were followed up one year, to compare the postoperative quality＇of life and living conditions. Results The blood loss, hospital stay, anal exhaust time and postoperative complications of the LAVH group were lower than the TAH group （ P 〈 0.05）, the quality of life and sexual life of LAVH group were better than TAH group. Conclusions Compared with abdominal hysterectomy, laparoscopic assisted vaginal hysterectomy reduce the damage for patients, shorten the hospital stay and has a broad prospect of clinical application.
