高频超声弹性成像与超声造影在甲状腺结节良恶性鉴别中的应用价值

摘    要:目的研究高频超声弹性成像与超声造影在甲状腺结节良恶性鉴别中的应用价值。方法选取我院2016年9月至2017年9月收治的甲状腺结节患者120例为研究对象,采用高频超声弹性成像、超声造影及其联合应用诊断甲状腺结节良恶性,比较不同检查方法的准确率。结果超声造影联合高频超声弹性成像检查的灵敏度、特异度、阳性预测值、阴性预测值及符合率均高于超声造影及高频超声弹性成像单独检查(P<0.05)。超声造影联合高频超声弹性成像检查的ROC曲线下面积大于超声造影及高频超声弹性成像单独检查(P<0.05)。结论高频超声弹性成像与超声造影技术对在鉴别甲状腺结节的良恶性方面均具有较高的诊断价值,两者联合检查可在一定程度上提高确诊率。

Abstract:Objective To study the application value of high frequency ultrasound elastography and contrast-enhanced ultrasound in differentiation of benign and malignant thyroid nodules.Methods A total of 120 patients with thyroid nodules admitted in our hospital from September 2016 to September 2017 were selected as the research objects, high frequency ultrasound elastography, contrast-enhanced ultrasound and the combination of the two methods were used to diagnose benign and malignant thyroid nodules.Accuracy of different methods were compared.Results The sensitivity, specificity, positive predictive value, negative predictive value and coincidence rate of contrast-enhanced ultrasound combined with high-frequency ultrasound elastography were higher than those of contrast-enhanced ultrasound and high-frequency ultrasound elastography alone(P<0.05).The area under ROC curve of contrast-enhanced ultrasound combined with high-frequency ultrasound elastography was larger than that of contrast-enhanced ultrasound and high-frequency ultrasound elastography alone (P<0.05).Conclusion High-frequency ultrasound elastography and contrast-enhanced ultrasound have high diagnostic value in differentiation of benign and malignant thyroid nodules.The combined examination of the two techniques can improve the diagnostic accuracy to a certain extent.
