2型糖尿病患者行脊柱手术后切口感染的危险因素分析

目的分析2型糖尿病患者行脊柱手术后发生切口感染的危险因素。方法回顾性分析行脊柱手术的441例2型糖尿病患者的临床资料,术后发生切口感染者21例(设为切口感染组),未发生者420例(设为非切口感染组)。分析导致患者术后发生切口感染的因素。结果 441例患者中,21例发生术后切口感染,发生率为4.76%。单因素分析显示,体质量、糖尿病病程、尿蛋白检验结果、脊柱手术史、器械置入及手术时间是引起2型糖尿病患者脊柱手术后发生切口感染的危险因素(P<0.05)。多因素Logistic回归分析显示,体质量、尿蛋白检验结果、手术时间是引起2型糖尿病患者脊柱手术后发生切口感染的独立危险因素(P<0.05)。结论引起2型糖尿病患者脊柱手术后发生切口感染的危险因素较多,临床应探寻合理的预防措施,以防止切口感染发生。

Analysis of risk factors for wound infection after spinal surgery in patients with type 2 diabetes mellitus

Objective To analyze the risk factors of wound infection after spinal surgery in patients with type 2 diabetes mellitus.Methods The clinical data of 441 patients with type 2 diabetes mellitus undergoing spinal surgery were retrospectively analyzed.Incision infection occurred in 21 patients (set as incision infection group), and did not occur in 420 cases(set as non-invasive infection group).The factors that lead to postoperative wound infection were analyzed.Results In the 441 patients, 21 cases occurred postoperative wound infections, with an incidence of 4.76%.The univariate analysis showed that body weight, duration of diabetes, urine protein test results, history of spinal surgery, instrument placement and operative time were risk factors for wound infection after spinal surgery in patients with type 2 diabetes mellitus (P<0.05).The multivariate Logistic regression analysis showed that body mass, urine protein test results and operation time were the independent risk factors for wound infection after spinal surgery in patients with type 2 diabetes mellitus(P<0.05).Conclusion There are many risk factors for incision infection in patients with type 2 diabetes after spinal surgery, reasonable preventive measures should be explored to prevent incision infection.
