血清胃蛋白酶原比值、癌胚抗原和糖类抗原19-9联合检测对胃良恶性溃疡的诊断价值

目的探讨血清胃蛋白酶原比值(PGR)、癌胚抗原(CEA)和糖类抗原19-9(CA19-9)对胃良恶性溃疡的诊断价值。方法选择2016年1月至2018年11月行胃镜及病理检查患者180例。根据胃镜及病理检查结分为胃溃疡组60例与胃癌组60例,另选同期行胃镜检查的非萎缩性胃炎组患者60例作为对照组。检测患者血清胃蛋白酶原(PG)Ⅰ、PGⅡ、CEA和CA19-9,计算PGR,并进行组间比较分析。结果与对照组和胃溃疡组比较,胃癌组患者的血清PGR显著下降(P<0.01),CEA和CA19-9显著升高(P<0.01)。胃癌组患者中血清PGR在胃癌发生转移患者均显著下降(P<0.05),CA19-9在胃癌发生转移患者显著升高(P<0.05)。联合检测敏感性升高(P<0.05)。结论联合检测血清PGR和CEA和CA19-9对胃良恶性溃疡诊断有较高价值。

Clinical significance of joint detection of serum pepsinogen,carcinoembryonic antigen and carbohydrate antigen 19-9 in diagnosis and prognosis of gastric cancer

Objective To investigate the diagnostic value of serum pepsinogen ratio(PGR),carcinoembryonic antigen(CEA) and carbohydrate antigen 19-9(CA19-9) for benign and malignant gastric ulcer.Methods From January 2016 to November 2018,180 patients who underwent gastroscopy and pathological examination were selected.According to gastroscopy and pathology,60 cases were in gastric ulcer group and 60 cases were in ulcerative gastric cancer group,and 60 cases of non-atrophic gastritis group were selected as control group.The serum pepsinogen(PG)I,PGII,CEA and CA19-9 levels were measured.PGR was calculated and compared between groups.Results Compared with the control group and the gastric ulcer group,the serum PGR of the ulcer-type gastric cancer group was significantly decreased(P<0.01),and the CEA and CA19-9 levels were significantly increased(P<0.01).The serum PGR level was significantly decreased in the patients with metastasis of gastric cancer(P<0.05);the serum CA19-9 level of the patients with metastasis of gastric cancer was significantly elevated(P<0.05).Joint detection increased sensitivity(P<0.05).Conclusions Combined detection of serum PGR and CEA and CA19-9 has high value in the diagnosis of benign and malignant gastric ulcer.
