慢性阻塞性肺疾病患者急性加重期C反应蛋白与纤维蛋白原及前白蛋白变化

摘    要:目的 探讨慢性阻塞性肺疾病急性加重期患者血清C反应蛋白、纤维蛋白原及前白蛋白的含量变化及临床意义.方法 采用速率散射比浊法分别检测30例慢性阻塞性肺疾病患者急性加重期、缓解期及30例体检者血清C反应蛋白、纤维蛋白原及前白蛋白的含量,同时测定其第1秒用力呼气容积占预计值百分比(FEV1%)及动脉血氧分压(PaO2).结果 ①慢性阻塞性肺疾病急性加重期及缓解期血清C反应蛋白及纤维蛋白原较对照组均明显升高,差异有统计学意义(F值分别为151.61和70.45,均P<0.05,q值分别为22.40,8.30;16.72,7.12;均P<0.05);而慢性阻塞性肺疾病急性加重期及缓解期血清前自蛋白较对照组均明显降低,差异有统计学意义(F=33.83,P<0.05,q值分别为15.26,6.31,P均<0.05).②急性加重期患者治疗后处于缓解期时C反应蛋白及纤维蛋白原均明显降低,差异有统计学意义(q值分别等于20.06及9.61,均P<0.05);而急性加重期患者治疗后处于缓解期时前白蛋白值明显升高(g=10.35,P<0.05).③血清C反应蛋白及纤维蛋白原水平与FEV1%均呈负相关(r分别等于-0.73及-0.82,均P<0.01);而血清前白蛋白水平与FEV1%均呈正相关(r=0.62,P<0.05).④血清C反应蛋白及纤维蛋白原水平与PaO2亦均呈负相关(r=-0.81及-0.68,均P<0.01);而血清前白蛋白水平与PaO2呈正相关(r=0.71,P<0.05).结论 血清C反应蛋白、纤维蛋白原及前白蛋白的检测对于慢性阻塞性肺疾病急性加重临床和治疗效果判断具有一定意义. 关

Abstract:Objective To investigate the changes of serum C-reactive protein (CRP), prealbumin(PA) and fibrinogen(Fbg) in patients of acute exacerbation of COPD(AECOPD).Methods The serum C-reactive protein,fibrinogen and prealbumin level were detected in 30 patients of AECOPD and COPD respectively and in 30 controls,meanwhile, their FEV1% and PaO2 also be measured.Results The serum CRP and Fbg level were apparently higher in the patients of both COPD and AECOPD than those in the controls (F=151.61 and 70.45 respectively,P<0.05 ;q = 22.40, 8.30, 16.72 and 7.12 respectively, P <0.05) ;while the PA level was lower in the patients of both AECOPD and COPD than that in the controls(F = 33.83, P < 0.05, q = 15.26 and 6.31, P <0.05).After treatment, the serum CRP and Fbg level in patients of AECOPD was significantly lower(q = 20.06 and 9.61 respectively, P <0.05) ; while the PA level was higher(q = 10.35, P <0.05).The serum CRP and Fbg level was negatively related with the FEV1% (r = - 0.73 and - 0.82 respectively, P < 0.01).While the PA level was positively related with FEV1% (r = 0.62, P < 0.05).The serum CRP and Fbg level was negatively related with PaO2 (r = -0.81 and -0.68 respectively, P< 0.01).While the PA level was positively related with PaO2 (r = 0.71, P<0.05).Conclusions The detection of serum CRP, Fbg and PA level may be useful for the diagnosis and treatment of patients of AECOPD.
