腹部外科手术合并糖尿病患者早期肠内营养治疗的护理研究

目的:探讨腹部外科合并糖尿病患者在术后早期肠内营养过程中的护理方法。方法:将100例腹部外科合并糖尿病患者随机分为观察组50例和对照组50例。术前均未接受肠外营养和肠内营养(EN)。对照组术后常规处理;观察组术后24 h经营养管泵注肠内营养糖尿病专用制剂,并将手术后引流出的部分胆汁、胃液经过滤后与EN营养液一并回输。比较两组患者术后肠功能恢复时间,术后血清白蛋白、血清前白蛋白、血清转铁蛋白的情况。结果:观察组在泵注肠内营养过程中无1例患者发生严重并发症,86%患者血糖稳定,控制在(7.8±1.2)mmol/L范围内。与对照组比较,观察组治疗后血清白蛋白、血清前白蛋白、血清转铁蛋白明显升高(P<0.05)。结论:对腹部外科疾病合并糖尿病患者术后早期应用糖尿病专用制剂是营养治疗的有效手段。治疗期间应采取科学有效的护理方法,加强基础护理,监测血糖变化,预防糖尿病和手术相关并发症的发生。

Objective：Discussion abdomen surgical department merge diabetic after the technique in early intestines in nutrition process nursing method. Methods ：100 patients were randomly divided into observation group and control group ,50 patients in each group. 24 h after alimentary tract pump note intes tines in nutrition diabetes special purpose preparation, around examination treatment each nutrition, biochemistry target change. Through a series of relat ed nursing measure, monitor and control blood sugar, prevention complication occurrence. Results：In the nutrition process the patient does not have the se rious complication 1 example in the pump note intestines ,86% patients blood sugar were stable, controls in （7.8 ：t： 1.2） mmol/L scope. After treatment, serum albumin, prealbumin, transferrin and other nutritional indices were significantly higher （P 〈 0.05 ）, body weight, hemoglobin, alanine aminotrans ferase, total bilirubin was not changed. Conclusion：After abdomen surgical department disease merge diabetic technique,the early application diabetes spe cial purpose preparation is the nutrition treatment effective measure. Treatment period should adopt the science effective nursing method, strengthens foun dation nursing, the monitor blood sugar change, prevents diabetes and the surgery related complication occurrence.
