腹腔镜与开腹胆囊切除术患者免疫功能的比较

目的探讨腹腔镜胆囊切除手术与传统开腹胆囊切除术对机体免疫系统的影响。方法对择期行胆囊切除的患者随机分成腹腔镜胆囊切除术组和开腹胆囊切除术组各60例。术前和术后采静脉血测定患者各项指标,包括总蛋白、白蛋白、血糖、白细胞计数、C反应蛋白浓度、T细胞亚群水平检测。结果腹腔镜胆囊切除术组总蛋白、白蛋白、血糖、C反应蛋白、白细胞计数各项指标手术前后水平相近(P>0.05),而开腹胆囊切除术组手术前后各项指标间差异有统计学意义(P<0.05)。开腹胆囊切除术组术后1、3、5dCD3+、CD4+、CD8+均明显低于腹腔镜胆囊切除术组(P<0.05)。结论与传统开腹胆囊切除术比,腹腔镜胆囊切除术对机体免疫功能影响较小。

Immune function comparison of Patients with Conventional or Laparoscopic Cholecystectomy

Objective To compare the immune responses in patients with conventional and laparoscopic cholecystectomy.Methods Sixty cholecystectomy patients were divided into two groups randomly:laparoscopic cholecystectomy(LC group,n=60)and open cholecystectomy(OC,n=60).The concentration of C reactive protein(CRP),blood glucose(Glu),total protein(TP)and albumin(AL)in serum were tested.The lymphocyte subsets of CD3,CD4,CD8,and NK cell cytotoxicity were measured.Results The levels of TP,Glu,CRP and WBC in LC group did not change before and after operation(P>0.05),but the levels of TP,Glu,CRP and WBC were lower after operation than those before operation in OC group with significant difference(P<0.05).After operation,the levels of CD3+,CD4+ and CD8+ were lower in OC group than those in LC group(P<0.05).Conclusion The damage of immune function caused by laparoscopic cholecystectomy is less than that by conventional cholecystectomy.
