宫颈DNA定量分析在宫颈病变筛查中的临床价值

目的探讨宫颈脱落细胞DNA定量分析在宫颈病变筛查中的临床价值。方法收集门诊及住院病例885例,同时行宫颈脱落细胞DNA定量分析及液基薄层细胞学(TCT)两种方法筛查宫颈病变,将其筛查结果进行对比分析。结果 885例病例中DNA定量分析方法筛查的阳性病例77例,其阳性检出率、病理诊断的符合率、阳性预测值、灵敏度和特异性,分别为8.70%、74.03%、96.61%,74.03%和75%;TCT法筛查的阳性病例34例,其阳性检出率、病理诊断的符合率、阳性预测值、灵敏度和特异性,分别为3.84%、70.59%,43.64%、70.59%、39.22%。结论 DNA定量分方法与病理诊断的符合率、阳性预测值、敏感度、特异度均高于液基薄层细胞学方法,是宫颈病变筛查的有效方法。

The clinical value of quantitative analysis of cervical lesions DNA in screening

Objective To explore the clinical value of quantitative analysis of cervical cells exfoliated DNA and TCT check in cervical cancer screening.Methods The clinical data of 885 cases in outpatient and hospitalization,with cervical brush materials,the TCT and DNA methods were used for cervical cancer screening at the same time.Results 885 cases cervical cells exfoliated quantitative analysis method of DNA screening positive cases in 77 cases,the positive rate of coincidence rate,pathological diagnosis,positive predictive value,sensitivity and specificity were respectively 8.70%,74.03%,96.61%,74.03% and 75%;TCT method of screening positive cases and example,its positive detection rate,the pathological diagnosis coincidence rate,the positive predictive value,sensitivity and specificity were respectively 3.84%,70.59%,43.64%,70.59% and 39.22%.Conclusion The pathological diagnosis coincidence rate,the positive predictive value,sensitivity,specific degrees of quantitative analysis of cervical lesions DNA in screening are higher than those of the liquid and thin layer cytology method.
