联合检测血浆和肽素与肌钙蛋白在急性心肌梗死诊断中的应用

目的:观察急性胸痛患者血浆和肽素(Copeptin)与肌钙蛋白(cTnI)水平的变化,寻找早期快速诊断急性心肌梗死(AMI)的方法。方法:收集急诊科2010年11月—2012年5月就诊的100例发病6h内的急性胸痛患者,其中AMI者48例,不稳定心绞痛32例,非心源性胸痛20例,进行血浆Copeptin及cTnI的定量测定。结果:AMI组血浆Copeptin水平较其他两组明显升高,差异有统计学意义(P<0.05),cTnI阴性AMI患者血浆Copeptin水平明显高于cTnI阳性AMI患者。结论:AMI患者早期Copeptin水平明显升高,血浆cTnI浓度变化与Copeptin水平呈负相关,故联合检测血浆和肽素与肌钙蛋白,有助于急诊科胸痛的排查,提高AMI早期诊断率。

Accomplication of joint detection of copeptin and cTnI in early diagnosis for patients with acute myocardial infarction

Objective:To observe the changes of copeptin and cardiac treponin I(cTnI) in plasma for patients with acute chest pain,inorder to find the way in early diagnosis for patients with acute myocardial infarction(AMI).Methods:A total of 100 acute chest pain patients occurred early(within 6 hours) admitted to the hospital emergency from Novermber 2010 to May 2012 were collected,include 48 cases of AMI,32 cases unstable angina,non-cardiac chest pain 20 cases,copeptin and cTnI in plasma were tested.Results:patients with AMI copeptin levels increased significantly compared with anyone group(P<0.05),the level of copeptin in cTnI-negative AMI group was obviously higher than cTnI-positive AMI group.Conclusion:The level of copeptin in patients with early AMI have been significantly high;the concentration changes between copeptin and cTnI in plasma is negative correlation;copeptin joint cTnI can be used as early diagnostic indicator of AMI,and can increase the rate of diagnosis of AMI.
