C反应蛋白定量测定诊断细菌性肠炎的准确性评价

目的探讨血清C反应蛋白测定诊断细菌性肠炎的准确性。方法选择某医院肠道门诊腹泻患者中经病原学检查确诊为细菌性肠炎的88例患者为病例组,大便轮状病毒抗原检测阳性的104例腹泻患者为对照组,用免疫比浊法测定两组患者血清C反应蛋白(CRP)浓度,以血清CRP≥8mg/L作为阳性判断标准,分析血清CRP检查诊断细菌性肠炎的准确性。结果细菌性肠炎组血清CRP浓度为(86.3±32.8)mg/L,病毒性肠炎组血清CRP为浓度为(5.5±3.4)mg/L,两组患者血清CRP浓度差异有统计学意义(t=24.97,P<0.01);血清CRP检查诊断细菌性肠炎的灵敏度为96.59%,特异度89.42%,漏诊率为3.41%,误诊率为10.58%,约登指数为0.90,阳性预测值为88.54%,阴性预测值为96.88%。结论血清CRP检查诊断细菌性肠炎具有较好的准确性。

Quantitative Evaluation of Bacterial Enteritis Diagnoslsed by C-reactive Protein

Objective Determination of serum C-reactive protein the accuracy of diagnosis of bacterial enteritis.Method Choose a hospital outpatients with diarrhea in the gut by pathogenic bacterial enteritis diagnosed 88 patients as case group,stool antigen rotavirus diarrhea in 104 patients tested positive for the control group patients,determined by immune turbidimetry two C-reactive protein serum（CRP） concentrations of serum CRP≥8mg/L as a positive standard,analysis of serum CRP check the accuracy of diagnosis of bacterial enteritis.Results Bacterial enteritis serum CRP concentration（86.3±32.8）mg/L,virus enteritis is the concentration of serum CRP（5.5±3.4）mg/L,serum CRP levels between the two groups was statistically significant（t=24.97,P0.01）;serum CRP diagnosis of bacterial enteritis sensitivity 96.59%,specificity of 89.42%,missed diagnosis rate was 3.41%,misdiagnosis rate was 10.58%,Youden index was 0.90,positive predictive value was 88.54%,negative predictive value 96.88%.Conclusion Serum CRP diagnosis of bacterial enteritis with good accuracy.
