摘    要:目的 观察慢性肾病对大鼠骨折愈合早期骨痂质量及生物力学的影响.方法 将40只雄性SD大鼠随机分成实验组及对照组,每组20只.实验组给予含0.75%的腺嘌呤饲料喂养建立慢性肾病大鼠模型,对照组给予正常饮食.4周后,两组大鼠于右侧胫骨中段制作开放骨折模型,克氏针内固定,术后予以正常饲料喂养.骨折造模术后4周,过量麻醉处死动物,切取右侧胫骨进行生物力学测试及Micro CT扫描.结果 骨折后4周,生物力学测试显示,实验组胫骨最大载荷(34.85±9.39)N]较对照组(46.42±3.26)N]降低24.9%(P<0.05);强度(47.65±5.64) N· mm]较对照组(103.79 ±4.12) N/mm]降低54.1%(P<0.05);能量吸收(7.17±.47)N·mm]较对照组(14.25±2.21)N ·mm]下降49.7%(P< 0.05).Micro CT分析显示实验组骨痂组织总量(18.45±2.11)mm3]较对照(29.60 ±2.45) mm3]组下降37.7%(P<0.05);骨性骨痂量(7.12±3.40)mm3]较对照组(15.65±1.32)mm3]下降54.50%(P< 0.05);骨体积分数(40.77±2.27)%]较对照组(44.65±2.12)%]下降8.9%(P<0.05).结论 慢性肾病对大鼠骨折愈合早期骨痂质量及力学强度均有不良影响.

Abstract:Objective To investigate the influence of chronic kidney disease on the process of fracture healing in rats. Methods 40 SD rats were divided into two groups: experimental group and normal control group. The experimental group was freely fed an adenine diet (0.75 g adenine in 100 g normal diet) for four weeks to establish CKD model. After 4 weeks, they were performed open fracture at the middle tibiae and fixed with Kirschner needles. Normal diet was given postoperatively. Four weeks after the surgery, all rats were killed with an overdose of Chloralic Hydras, the right tibiae were harvested by surgery to undergo miero-CT scan and biomeehanical test. Results Maximum load (34.85±9.39) N] in experimental group was decreased 24.9% than that in normal control group (46.42±3.26) N] (P 〈 0.05); stiffness in experimental group (47.65±5.64) N/mm] was decreased 54.1% than that in normal control group (103.79±4.12) N/mm] (P 〈 0.05); energy uptake in experimental group (7.17±4.47) N-mm] was decreased 49.7% than that in normal control group (14.25±2.21) N'mm] (P 〈 0.05). Micro CT analysis showed that the TV in experimental group (18.45±2.11) mm3] was decreased 37.7% than that in normal control group (29.60±2.45) mm3] (P 〈 0.05); BV in experimental group (7.12±3.40) mm3] was decreased 54.50% than that in normal control group (15.65±1.32) mm3] (P 〈 0.05); BV/TV in experimental group (40.77±2.27)%] was decreased 8.9% than that in normal control group (44.65±2.12)%] (P 〈 0.05). Conclusion Chronic kidney disease has a harmful effect on the early process of fracture healing in rats.
