彩色多普勒超声在乳腺检查中的诊断价值及临床应用

摘    要:目的 探讨彩色多普勒超声在乳腺检查中的准确度及临床效果.方法 选取2008年3月~2012年7月在浙江省慈溪市坎墩医院就诊并做彩色多普勒乳腺超声检查的患者34例作为观察组,同时选取同期做乳腺钼靶X线检查的患者34例作为对照组,分析两种检查方式与手术病理结果的对照.结果 对照组34例患者,经手术及病理证实为良性乳腺肿块20例,其中,乳腺囊肿9例,乳腺纤维腺瘤5例,乳腺脂肪瘤4例,乳腺导管内乳头状瘤2例;恶性肿瘤4例,其中,髓样癌1例,导管原位癌2例,浸润性导管癌1例;非肿瘤10例.观察组34例患者,经手术及病理证实为良性乳腺肿块20例,其中,乳腺囊肿9例,乳腺纤维腺瘤5例,乳腺脂肪瘤3例,乳腺导管内乳头状瘤3例;恶性肿瘤5例,其中,髓样癌1例,导管原位癌2例,浸润性导管癌2例;非肿瘤9例.观察组良、恶性乳腺肿块的大小、病灶边界、病灶形态、内部回声、有无钙化灶等超声声像图表现差异无统计学意义(P>0.05).对照组乳腺钼靶X线初诊共诊断出良性乳腺肿块患者12例,确诊率为60.0%;恶性肿瘤患者7例,确诊率为57.1%;非肿瘤患者15例,确诊率为66.7%.观察组彩色多普勒超声初诊共诊断出良性乳腺肿块患者21例确诊率为95.2%;恶性肿瘤患者5例,确诊率为100.0%;非肿瘤患者8例,确诊率为88.9%;两组确诊率差异有统计学意义(P<0.05).结论 彩色多普勒乳腺超声检查具有较高的检查准确度,值得在乳腺疾病患者检查中推广应用.

Abstract:Objective To investigate the accuracy and clinical effect of color Doppler ultrasound in breast examination. Methods 34 patients who were given color Doppler breast ultrasound in Kandun Hoapital of Cixi City from March 2008 to July 2012 were selected as the observation group, at the same period, 34 patients who were taken mammography X-ray examination were selected as control group, the two inspection methods and surgical and pathological findings were analyzed. Results 34 patients of the control group, 20 cases were confirmed by surgery and pathology as benign breast lumps, among whom, 9 cases of breast cysts, 5 cases of breast fibroadenoma, 4 cases of breast lipoma, 2 cases of intra- ductal papilloma; 4 cases were confirmed as malignant tumor, among whom, 1 case of medullary carcinoma, 2 cases of ductal carcinoma in situ, 1 case of invasive ductal carcinoma; 10 cases were confirmed as non-tumor. 34 patients of observation group, 20 cases were confirmed by surgery and pathology as benign breast lumps, among whom, 9 cases of breast cysts, 5 cases of breast fibroadenoma, 3 cases of breast lipoma, 3 cases of intraductal papilloma; 5 cancer cases were confirmed as malignant tumor, among whom, 1 case of medullary carcinoma, 2 cases of ductal carcinoma in situ, 2 cases of invasive ductal carcinoma; 9 cases were confirmed as non-tumor. The ultrasonography such as benign and malignant breast tumor size, lesion boundary, lesion morphology, internal echo, with or without calcification of observa- tion group were found had no statistically significant difference (P 〉 0.05). 12 cases of benign breast lumps were diag- nosed in control group, diagnosis rate was 60.0%; 7 cases of patients were malignant tumors, diagnosis rate was 57.1%; non-cancer patients were 15 cases, diagnosis rate was 66.7%. 21 cases of benign breast lumps were diagnosed in ob- servation group, diagnosis rate was 95.2%; 5 cases of patients with malignant tumors, diagnosis rate was 100.0%; 8 cases of non-cancer patients, the diagnosis rate was 88.9%; the confirmed rate differences of two groups were statis- tically significant (P 〈 0.05). Conclusion Color Doppler breast ultrasound has a high inspection accuracy, it is worthy of popularization and application in the examination of patients with breast disease.
