胃癌术后早期肠内营养与肠外营养的疗效比较

目的 比较胃癌患者在术后早期采用肠内营养支持和肠外营养支持在疗效上的差别.方法 将浙江绍兴第二医院于2009年11月～2012年11月收治的60例胃癌患者作为研究对象,按照1：1的比例分为实验组和对照组,实验组患者在实施胃癌根治术后采用肠内营养支持方式治疗,对照组患者在实施胃癌根治术后采取肠外营养支持进行治疗,比较分析两种不同支持方法之间的临床效果.结果 实验组术后首次下床活动时间为（3.17±1.67）d,首次经口进食时间为（3.34±1.42）d,拔除胃管时间为（1.12±0.98）d,拔除腹腔引流管时间为（2.67±0.93）d,拔除尿管时间为（4.65±1.75）d,白蛋白值为（39.7±3.7）g/L,前白蛋白值为（228±19）mg/L,血红蛋白值为（125.21±29.15）g/L,实验组各项指标均大幅优于对照组,差异有统计学意义（P＜0.05）.结论 胃癌根治术后对患者实施肠内营养支持相较于肠外营养支持更加有效、经济和安全,值得临床推广.

Comparison of therapeutic effects of enteral nutrition and total parenteral nutrition in early postoperative gastric cancer

Objective To compare and study the differences in efficacy of early gastric cancer patients after surgery using enteral nutrition and parenteral nutrition support.Methods 60 cases of gastric cancer patients treated in Shaoxing Second Hospital of Zhejiang Province from November 2009 to November 2012 were as research subjects,in accordance with the ratio of 1∶1,they were divided into experimental group and control group,the experimental group was taken the enteral nutrition support method after implementation of radical gastric cancer surgery,the control group were taken the parenteral nutrition support method after implementation of radical gastric cancer surgery.The clinical effect of two different methods was compared and analyzed.Results In the experimental group,the postoperative first ambulation time was （3.17±1.67） d,the first feeding time was （3.34±1.42） d after opening,cstuation time was （1.12±0.98） d,drainage time was （2.67±0.93） d,catheter removal time was （4.65±1.75） d,albumin value was （39.7±3.7） g/L,prealbumin value was （228±19） mg/L,hemoglobin value was （125.21±29.15） g/L,the indicators above were significantly better than the control group,with significant differences （P ＜ 0.05）.Conclusion Enteral nutrition for patients after taking radical gastrectomy compared to parenteral nutrition is more efficient,economical and safe,which is worthy of promotion.
