缺血性脑卒中患者血尿酸的表达及危险因素分析

摘    要:目的 分析缺血性脑卒中患者血尿酸(UA)表达及危险因素.方法 145例缺血性脑卒中患者为病例组,同期健康体检者123例为对照组,比较病例组与对照组血UA水平及其他危险因素的差别,Logistic回归分析这些因素与缺血性脑卒中发病的关系.病例组按照不同发病年龄段及发病时期分别分为3组,比较各组间血UA水平的差异.结果 病例组中糖尿病、高血脂患者例数及血UA水平高于对照组.多因素Logistic回归分析显示,糖尿病、高血脂及血UA水平与缺血性脑卒中发生密切相关(P<0.05).>60岁年龄组与<45岁及45 ~60岁组比较,血UA水平较高(345.73 ±40.09) μmol/L],组间比较差异有统计学意义(P<0.05).发病时期<1 d组与1~<4 d及4~7 d组比较,血UA水平较高(351.08±52.41) μmol/L],组间比较差异有统计学意义(P<0.05).结论 血UA水平升高可能是缺血性脑卒中患者发病的危险因素,并且随着发病年龄的增长,血UA水平增高;随着缺血性脑卒中病情进展,血UA水平降低.

Abstract:ObjectiVe To investigate the serum uric acid levels and the related risk factors in ischemic stroke patients. Methods Serum uric acid levels and related risk factors were compared between 145 ischemic stroke patients (trial group) and 123 normal controls, which were further analyzed by Logistic regression modeling. The trial group were divided into 3 subgroups respectively according to different ages and onset period. Results In the group of ischemic stroke patients, the cases of diabetes, hyperlipidemia and the levels of uric acid were significantly higher than those in the control group, which were closely related to the development of ischemie stroke (P 〈 0.05) showed by Logistic regression modeling. The age 〉60 patients group had a higher uric acid level (345.73±40.09) μmol/L] than age 〈45 and 45-60 group, the differences were statistically significant (P 〈 0.05). The onset period less than 1 day patients group had a higher uric acid level (351.08±52.41) μmol/L] than 1-〈4 days and 4-7 days group, the differences of two were statistically significant (P 〈 0.05). Conclusion The elevated serum levels of uric acid may be risk factor for ischemic stroke. The levels of uric acid increased with the growth of patients" age, and decreased with disease progress.
