牙挺多点增隙法拔除下颌水平阻生第三磨牙的研究

目的探讨牙挺多点增隙法在下颌阻生第三磨牙拔除术中的应用。方法下颌水平阻生第_5-磨牙低位120颗,中位阻生128颗．均随机分为A、B组,A组采用牙挺多点增隙法;B组采用传统牙挺法。分别观察拔牙用时、术中去骨、断根、术后疼痛、术后并发干槽症等指标,并进行对比分析。结果A组患者在拔除下颌水平低位阻生第三磨牙时,在手术用时、去骨要求上明显低于B组,差异有统计学意义（P〈0．05）。A组患者在拔除水平中位阻生第三磨牙时,术中断根明显少于B组,差异有统计学意义（P〈0．05）,两组其他方面无显著性差异。结论采用多点增隙法比传统牙挺法手术用时少,手术创伤减少,断根发生率明显降低,符合微创原则,值得推广应用．．

Extracting impacted mandibular third molars by dental elevator multiple points socket enlargement

Objective To evaluate the method of extracting impacted horizontal mandibular third molars by dental elevator multiple points socket enlargement. Methods Two hundred and forty eight horizontal impacted mandibular third molars were randomly divided into two groups： patients in group A were treated with MPEM, patients in group B were treated with the dental elevator by the traditional method （TM）. Each group included 120 position C teeth （PC） and 128 position B teeth （PB）. The time spent in the surgery, the removal of bone, fractured roots, postoperative pain, and dry socket were observed. The data was analyzed with SPSS 13.0 software package. Results In the removal of PC, the time spent in the surgery and the removal of bone of the extraction in group A was significantly lower than those in group B （ both P 〈 0.05 ）. In the removal of PB, the rate of fractured roots in group A was significantly lower than that in group B （ P 〈 0. 05 ）. Conclusion MPEM can reduce the time spent in the surgery, the excised bone and fractured roots of the extraction, so it is a suitable method for extraction of complicated tooth.
