可降解抗生素缓释载体治疗慢性骨髓炎动物模型的实验研究

摘    要:目的通过动物模型实验研究可降解缓释抗生素载体对慢性骨髓炎的治疗效果。方法40只新西兰大白兔通过葡萄球菌接种经4周时问演变成为慢性股骨骨髓炎模型,随机分为4组,每组10只。A组病灶植入万古霉素一明胶海绵;B组植入万古霉素一硫酸钙;C组病灶内植人万古霉素一纳米羟基磷灰石,D组单纯植入万古霉素粉剂0.1g,分别于术后5d,15d及30d测量病灶分泌物抗生素药物浓度,同时取病灶局部病理检查进行分析比较。结果术后30d取病灶组织,B组及c组20只白兔有18例术后30d病理切片白细胞数明显减少,并有明显的新生肉芽组织q三成,而A组及D组20只白兔术前及术后对照白细胞数量未见明显减少。病灶引流液药物浓度A组及D组数据均呈现在植入后爆发性释放,很快便急剧下降,失去治疗作用。B组及c组引流液药物浓度呈现缓慢持续且有效的释放规律,至术后30d均值仍高于最低有效浓度。结论可降解缓释抗生素载体对慢性骨髓炎治疗有效,各种载药体具有不同的生物学特性,值得进一步深入研究探讨。

Animal Study of Degradable Antibiotics Carrien in the Treatment of Chronic Osteomyelitis

Abstract:Objective To study the therapeutic efficacy of degradable antibiotics' carrier animal model study. Methods In 40 albino rabbits were developed into chronic osteomyelitis with staphylococcus in femur and were divide into 4 groups in radom. A-gyoup :vancomycin gelatin sponge planted in its focus of infection. B-group :vancomycin-Ca- SO4 ;C group:vancomycin nanometeric HAC and the D group :used by vancomycin dusts only. To survey the antibi- otics level of the secreta and check up the pathobiologic slices of the focus of infection in the 5 days,15 days and 30 days after opration. Results The quantities of leucocytes had not reduced obviously both in the unit A and D. The quantities of leucocytes had reduced obviously in 18 albino rabbits of the unit B and C,and the new granulation tissue had been developed obviously in the unit B and C. The drugs level of the secretion changed with time and the unit A and D had not statistically significant difference. The difference between unit A and unitB,C is statistically significant. Conclusion Sustained-release antibioticsr currber is useful in the treatment of chronic osteomyelitis.
