益生菌干预下溃疡性结肠炎结肠组织中防御素的表达及意义

摘    要:目的 研究益生菌治疗溃疡性结肠炎(UC)前后防御素(HNP1-3)在结肠组织中的表达水平,明确益生菌对UC患者的治疗作用,探讨防御素在UC发病机制中的作用以及益生菌治疗UC的作用机制。方法30例UC患者,随机分成A、B、C三组,A组口服柳氮磺胺吡啶和美常安,B组口服柳氮磺胺吡啶和丽珠肠乐,C组口服柳氮磺吡啶,疗程均为1个月。于治疗前后各取病变肠组织,用免疫组化法检测其中HNP。的表达水平。结果A、B、ci组治疗前病变肠组织中HNP。均为阳性表达,治疗后均为阴性或弱阳性表达,各组治疗前后差异有统计学意义(P〈0.01)。A、B两组肠组织中HNP。的表达在治疗前后下降幅度比C组大,其差异有统计学意义(P〈0.01),而A、B两组服药前后下降幅度比较,其差异无统计学意义(P≥0.05)。结论HNP。参与UC的发生发展,能反映疾病的炎症程度;益生菌能有效降低UC患者的HNP1-3表达水平;HNP1-3可能与益生菌治疗UC的作用机制有关。

Abstract:Objective To determine the level of defensins ( HNP1-3 ) in colon tissues of ulcerative colitis before or after taking the probiotics, identify the therapeutical effect of probiotics in patients of UC and to explore the ruction of probiotics in the mechanisms of UC. Methods Choose 30 patients with UC, took them into A, B, C groups at random. Patients in A group took Sulfasalazine, Live Combined Bacillus Subtilis and Enterococcus Faecium Enteric-coated Capsules for a month. Patients in B group took Sulfasalazine and bifidobacteria for a month. Patients in C group took only Sulfasalazine for a month. The levels of HNP1_3 in colon mucosa of UC patients were detected by immunohistochemistry method before and after the treatment. Results HNP1-3 in colon lesion was positive before treatment while weakly positive or negative after treatment. The differences between before treatment and after treatment were significant in three groups (P 〈 0.01 ). And the differences of the levels of HNP1-3 between before and after the treatment in A and B groups were greater than C group (P 〈0.01). But no difference existed between A and B group (P≥0.05). Conclusion The induction of HNP1-3 is involved in process of inflammation and development of UC. The level of HNP1-3 indicates the severity of UC. The probiotics decreases the level of HNP1-3 effectively. HNP1-3 is likely to be related to the therapeutic mechanism of probiotics in UC patients.
