急性缺血性中风病中医综合治疗方案疗效及卫生经济学评价

目的 探讨急性缺血性中风病中医综合治疗方案的疗效及卫生经济学评价,为制定有效、经济、合理的治疗方案提供参考.方法 将研究对象分为中医治疗组和西医对照组,疗效观察指标为两组患者治疗前后神经功能缺损程度（NIHSS）评分、日常生活能力Barthel（MBI）指数评分、运动功能（FMA）评分、认知功能（MMSE）评分、中风病人生活质量（SS QOL）评分等.运用卫生经济学中的成本效果分析方法进行疗效及经济学评价.结果 临床疗效：治疗组在FMA 严重程度、Barthel 指数改善均优于对照组.卫生经济学评价：两组患者在药费、住院总费用方面有统计学意义（P 〈0.05）,治疗组药费、住院总费用低于对照组;住院费、检查费、治疗费等两组无统计学意义（P 〉0.05）.NIHSS 评分每减少1 分,ADL 评分每增加1 分,FMA 评分每增加1 分,MMSE 评分每增加1 分,SSQOL 评分每增加1 分,治疗组所需花费均较对照组少.结论 中医综合治疗方案对于治疗急性缺血性中风病具有显著的疗效,本方案较对照组在达到最佳的治疗效果同时费用使用更加合理,该方案经济、合理、有效.

Effects and Health Economic Evaluation of Acute Ischemic StrokeTreated with Comprehensive Regimen of Traditional Chinese Medicine

Objective To explore the effects and health economic evaluation of acute ischemic stroke treated with comprehensiveregimen of traditional Chinese medicine（TCM）.Methods The patients with acute ischemic stroke were divided into treatmentgroup treated by TCM treatment and control group by western medicine treatment.The NIH Stroke Scale （NIHSS）,Modified BarthelIndex （MBI）,Fugl Meyer Assessment（FMA）,Mini Mental State Examination （MMSE）and Stroke Specific Quality of Life Scale （SSQOL）were observed.The effects and health economic evaluation were assessed by cost efectiveness analysis（CEA）.ResultsThe FMA and MBI in treatment group was better than that in control group（P 〈0.05）.There was no statistical significance in the totalcosts for medicine and hospitalization between two groups（P 〈0.05）.The total costs for medicine and hospitalization in treatmentgroup was lower than that in control group.Conclusion The comprehensive regimen of TCM for acute ischemic stroke couldimprove the effects,and be more economic.
