贲门切缘癌残留与病理生物学行为及手术径路的关系

目的　探讨贲门癌口侧切缘癌残留与病理因素及手术径路的关系。方法　通过对 15 2例贲门癌病理生物学行为及手术径路与口侧切缘癌残留的回顾性调查 ,分析切缘癌残留的影响因素。结果　贲门癌的大体类型、分化程度、浸润深度、生长方式、淋巴结转移及手术径路的不同与口侧切缘癌残留均有明显统计学差异 (P <0 .0 5 )。结论　贲门癌的生物学行为是指导手术切除范围的重要基础。术前对其进行正确估计、选择最佳手术径路、充分切除癌灶是减少和防止口侧切缘癌残留的关键

The relationship between residue of carcinoma of gastric cardia at the incisal margin and pathological biological behavior and operative approaches

Objective To discuss the relationhsip between the residue of carcinoma of gastric cardia and the pathological factors and operative approaches.Methods The factors of the oral incisal margin residue were analyzed by investigating retrospectively pathological biological bahavior of carcinoma of gastric cardia and approaches and residue in 152 cases.Results There were significant correlations between biological behavior macroscopic types,histological differenctiation,invasive depth,growth pattern,lymphatic node metastasis, and operative approaches and proximal margin residue(P<0.05).Conclusion Resected extent of carcinoma of gastric cardia is based on biological behavior.To prevent poximal marginal residue,biologic behavior should be determined accurately before operation,optimal approaches should be chosen,and the lesion should be resected adequately.
