不同病理类型胃息肉流式细胞DNA定量研究

摘    要:应用流式细胞计对57 例正常胃粘膜、不同病理类型的胃息肉和胃癌组织作DNA 定量分析。正常胃粘膜、炎性息肉、增生性息肉、腺瘤性息肉及胃癌中,DNA 非整倍体检出率分别为0% 、7.7% 、9.1% 、36.4% 、58.3% ;各型胃息肉和胃癌组织的增殖期细胞比率均显著地高于正常胃粘膜组(P< 0.01);增生性或炎症性息肉组增殖期细胞比率与胃癌组比较,差异有显著性(P< 0.05)或非常显著性(P< 0.01),而腺瘤性息肉组增殖期细胞比率与胃癌组近似(P> 0.05)。结果表明胃息肉是一种细胞增殖活跃性病变,其中腺瘤性息肉的DNA 生物学行为更接近于胃癌,可能容易癌变;应用流式细胞DNA 定量分析技术,结合组织病理学诊断,能对胃息肉的预后作出更为确切的判断

Abstract:AIMS:To explore further the DNA biological features of different types of gastric polyps histopathologically and their relationships with gastric cancer. METHODS:The quantitative analysis of DNA was done with flowcytometry for 57 gastric specimens including normal gastric mucosa,different types of gastric polyps and gastric cancer. RESULTS:DNA aneuploid percentage was 0% in normal gastric mucosa,7.7% in inflammary polyps,9.1% in hyperplastic polyps,36.4% in adenomatous polyps and 58.3% in gastric cancer;There were statistically differences in percentage of S and G 2 M phase cells between different types of gastric polyps or gastric cancer and normal gastric mucosa ( P <0.01);Percentage of S and G 2 M phase cells in gastric cancer was significantly hypher than that in inflammary polyps or hyperplastic polyps ( P <0.05 and P <0.01),but no statistical differences were found between adenomatous polyps and gastric cancer ( P >0.05). CONCLUSIONS:Gastric polyps is a hyperplastic lesion in gastric mucosa and adenomatous polyps is closely associated with gastric cancer.DNA quantitative analysis with flowcytometry is an useful method in judging prognosis of gastric polyps.
