妊娠高血压综合征患者剖宫产术的安全性与临床效果研究

目的　探讨妊娠高血压综合征 (妊高征 )患者剖宫产术的安全性及手术效果。方法　采用历史前瞻性流行病学方法进行回顾性分析。结果　妊高征患者剖宫产终止妊娠率为 4 3.75 % ,其中重度妊高征患者剖宫产率为 97.37%。孕产妇病死率为 0 .6 4 % ,围生儿病死率为 1.83%。剖宫产术后 1周、术后 4 2天和术后 2年血压仍高于正常者分别为 18.5 8%、7.6 9%和 1.92 % ,术后 1周尿蛋白异常者为 2 .92 % ,但术后1年内均转为阴性。产褥病死率为 6 .0 1%。结论　术前经解痉等积极治疗与适时终止妊娠是减少母婴并发症 ,降低围生期病死率 ,提高生活质量的关键 ,对于重度妊高征患者 ,以剖宫产方式终止妊娠为宜。

Study on the safety and the clinical effect of cesareen section in pregnancy induced hypertension

Objective To explore the safety and effect of cesareen section in pregnancy induced hypertension(PIH).Methods A retrospectively epidemiologic study was conducted.Results The average rate of cesarean section in PIH was 43.75%,while it was 97.37% for moderate and severe PIH.The mortality of maternity was 0.64% ,which of fetus was 1.83%,18.58%,7.69% and 1.92% of cases still had hypertension 1 week,42 days and 2 years after cesarean section,respectively.There were 2.92% of patients with proteinuria one week after cesarean section,who became negative within one year.Puerperal morbidity was 6.01%.Conclusion Timely cesarean section with active antispastic treatment before operation is a key to reducing the complications and perinatal morbidity and improving life quality,especially for severe PIH.
