摘    要:目的 探讨64排螺旋CT诊断主动脉夹层的临床应用价值.方法 对32例主动脉夹层患者进行64排螺旋CT增强扫描,采用多平面重建、曲面重建、最大密度投影和容积再现等多种后处理技术,并结合原始横断面图像进行综合分析.结果 32例患者中:Ⅰ型8例(25%),Ⅱ型6例(18.8%),Ⅲ型18例(56.2%).32例AD患者都能清晰显示主动脉夹层的不同密度的真假腔以及分隔真假腔的内膜瓣.结合后处理重建图像,所有病例均显示真假腔之间各种形态的低密度内膜片及内膜破口的位置,纵行向下的真腔夹层延伸范围.结论 64排螺旋CT是一种安全、快捷、无创、有效的检查方法,对主动脉夹层的诊断、分型以及治疗方案的选择,术前、术后评估等方面具有重要的临床应用价值.

Abstract:Objective To investigate the clinical value of 64-slice spiral CT in diagnosing aortic dissection.Methods Thity-two cases with aortic dissection were scanned with enhanced 64 slice spiral CT.The initial data was reconstructed by multi-planar reformation,curved planar reconstruction,maximum intensty projection ,volume rendering and were generally analyzed with original axial images.Results Of the 32 cases,8 were typeⅠ aortic dessection(25%),6 cases type Ⅱ(18.8%) and 18 cases type Ⅲ(56.2%).The true and false cavity of the different density,the intimal flap seperating the true and false cavity in all of the 32 cases were showed clearly.By Combining the reconstructed images with all kinds of management technology,a the low density intimal flap of the different form between the true and false cavity,location of intimal rupture and the longitudinal downward range of the true cavity dissection in all the cases were showed.Conclusion Using 64-slice spiral CT is a safe,quick,non-invasive and effective method in diagnosis.64-slice spiral CT has an important clinical applicable value in all aspects,such as diagnosis of aortic dissection,accurate classfication,selection of therapy plan and the preoperative and postoperative evaluations.
