急性胰腺炎伴脂肪肝的临床观察

目的分析急性胰腺炎伴脂肪肝的临床表现。方法105例急性胰腺炎均行肝、胆、胰、脾CT扫描。均行血/尿淀粉酶检查。9例查血甘油三酯。结果105例急性胰腺炎中伴脂肪肝70例,占66.7%,其中Ⅰ级脂肪肝36例,Ⅱ级脂肪肝24例,Ⅲ级脂肪肝10例。伴胆系疾病者19例,占18.1%。不伴脂肪肝和胆系疾病者15例,占14.3%。既伴脂肪肝又伴胆系疾病者1例,占1%。9例查血甘油三酯,其中6例不同级别脂肪肝的甘油三酯不同程度的升高。16例在治疗过程中复查CT,随着急性胰腺炎得到控制,脂肪肝也有所好转。结论急性胰腺炎多伴脂肪肝,其原因可能是病人血中的甘油三酯升高引起的。

The Clinical Features of Acute Pancreatitis and Fatty Liver

Objective To analyze the clinical features of acute pancreatitis and fatty liver.Methods 105 cases with acute pancreatitis were examined by CT scanning.All patients had been done with amylase tests,among which,9 had been tested with triglyceride.Result 66.7%(70 cases) of 105 acute pancreatitis were accompanied with fatty liver,36/70 were grade Ⅰ,24/70 were grade Ⅱ,10/70 were grade Ⅲ.18.1%(19 cases) acute pancreatitis patients had biliary tract diseases,14.3%(15 cases) had no fatty liver and biliary tract diseases,1%(one case) had both fatty liver and biliary tract diseases.Among 9 cases with triglyceride test,6 had different grades of fatty liver and high value of triglyceride.16 of them showed better condition of acute pancreatitis and fatty liver in follow-up CT studies after treatment.Conclusion The cases of acute pancreatitis were often accompanied with fatty liver on base of increased level of triglyceride in the serum of patients.
