699例尿路感染病原菌分布及药敏试验分析

摘    要:目的探讨尿路感染病原菌和耐药性的变化。方法分析我科699例尿液培养阳性的尿路感染病原菌分布及其对抗菌药敏感性。结果革兰氏阴性杆菌占75.59%,其中大肠埃布菌60.33%;革兰氏阳性球菌占24.41%,其中肠球菌属占15.67%,真菌占1.39%;大肠埃布菌对氨苄西林的耐药率最高(80.63%);对阿米卡星的耐药率明显低于庆大霉素(P〈0.01),对喹诺酮类耐药率为41.45%~73.60%,对复方新诺明耐药率达60.10%。结论大肠埃希布菌仍是尿路感染的主要病原菌,肠球菌属、变形杆菌、克雷伯菌属、铜绿假单胞菌等病原菌比例有所增加,这些病原菌耐药性高,尤其氨苄西林、喹诺酮类、复方新诺明耐药性高,应注意根据药敏试验调整用药。

Abstract:Objective To detect the distribution of pathogenic bacteria of urinary tract infection and the their resistance to drugs. Methods To analysis the pathogenic bacteria distribution and their sensitivity to antibiosis drug from 699 cases with urinary tract infection conformed by urine culture. Results The proportion of Gram-Negative bacillus was 75.59% ( Escherichia Coli account for 60.33 % ) ; the proportion of Gram-positive cocco bacterium was 24.41% ( enterococcus account for 15.67 %, fungus for 1.39% ). The drug resistance rate of Escherichia Coli to Ampicillin was the highest ( 80.63% ) , to quinolones was from 41.45% to 73.60% , and to SMZ-TMP was 60.10%. The drug resistance rate to amikacin was clearly lower than that to gentamitin ( P 〈 0.01 ). Conclusions Escherichia Coli is still the main pathogenic bacteria of urinary tract infection. The proportion of enterococcus, bacillus proteus ,klebsiella, pseudomonas aeruginosa is raising with high drug resistance, expecially to Ampicillin, quinolones and SMZ-TMP. Some pathogenic bacteria have drug resistance to many types of drugs. Drugs should be adjusted according to drug sensitive test.
