体重指数与初诊2型糖尿病代谢状态及慢性并发症的相关性研究

摘    要:目的探讨体重指数与初诊2型糖尿病患者代谢状态及慢性并发症的关系。方法选取2010年1月至2011年12月我院初诊2型糖尿病患者82例作为研究对象。根据体重指数的不同将所有患者随机分为两组,每组各41例。分析比较两组患者空腹血糖及餐后血糖、血脂、血压、抵抗指数及并发症患病率。结果肥胖组的TC、TG、LDL和UA均大于正常组,而HDL低于正常组,两组在以上几个指标问的差异均具有显著性(P〈0.05)。肥胖组视网膜病变、冠心病、高血压、神经病变和肾脏病变的发生率均高于正常组,其中两组在冠心病、高血压和肾脏病变的差异有显著性∽〈0.05)。结论肥胖的初诊2型糖尿病患者更易发生代谢紊乱和并发慢性并发症,故对肥胖的患者要指导其合理饮食、适当运动,以延缓或避免糖尿病并发症的发生。

Abstract:Objective To investigate the relations among body mass index (BMI) with metabolic status and chronic complications in newly diagnosed type 2 diabetic patients. Methods 82 type 2 diabetes patients treated in our hospital from January 2010 to December 2011 were enrolled as the research objects. All patients were randomly divided into two groups according to the different BMI, 41 cases in each group. Then we analyzed and compared the fasting blood-glucose and postprandial blood sugar, blood fat, blood pressure, resistance index and complication rates of two groups. Results The TC, TG, LDL and UA of the obesity group were higher than those of the normal group, and HDL was lower than that of the normal group, the difference between the two groups were statistically significant (P〈0.05). The retinopathy, coronary heart disease, high blood pressure, neu- ropathy and kidney disease incidence of obesity group were higher than those in the normal group, coronary heart disease, hypertension and kidney between the two groups were statistically significant (P〈0.05). Conclusions Obesity in newly diagnosed type 2 diabetes patients was more susceptible to metabolic disorders and concurrent chronic complications. So more diet and exercise guidings were needed to the obese patients, to delay or prevent diabetes complications.
