彩色多普勒检测肺动脉血流对诊断室间隔缺损部位的临床价值

摘    要:目的探讨用彩色多普勒血流显像检测肺动脉的血流速度,对判断不同类型的室间隔缺损的临床价值。方法对经手术证实的76例先天性室间隔缺损患者,按缺损部位分为A、B两组。按缺损大小分为C、D两组。用彩色多普勒超声测量各房室内径、肺动脉内径、肺动脉收缩期的最大血流速度、室间隔缺损大小、室间隔缺损处左向右的最大分流速度。结果A组和B组在左房室内径、肺动脉内径无明显差异(P>0.05),肺动脉收缩期最大血流速度,两组有明显差异(P<0.05)。C组和D组在左房室内径、肺动脉内径、肺动脉血流速度有明显差异(P<0.05),室间隔分流速度无明显差异(P>0.05)。结论肺动脉收缩期最大血流速度对诊断室间隔小缺损的部位有一定的辅助作用。

Abstract:Objective To explore the clinical value of color Doppler blood flow display in diagnosing different kinds of ventricular septal defect by detecting the blood velocimeters of the pulmonary artery. Methods A total of 76 patients who had been confirmed by surgery were distributed as A, B two groups on the basis of the different sites of ventricular septal defect and as C. D two groups on the basis of the different sizes. The inner diameters of the atriam, ventricles the pulmonary artery and the sizes of ventricular septal defect the maximum velocities of the pulmonary artery in systolic phase and of the ventricular septal defect from left to right were detected by color Doppler. Results Between group A and B, there were no apparent differences in the inner diameters of the left atrium, left ventricle and the pulmonary artery, but significant differences were found in the maximum blood velocity the pulmonary artery in systolic phase. Between group C and D, there were significant differences in the inner diameters of the left atrium and left ventricle, but the blood velocity of the pulmonary artery, had no significant difference in the ventricular septal shunts. Conclusions The maximum velocity of the pulmonary artery in systolic phase has some subsidiary effect on the diagnosis of ventricular septal defect sites.
