针刺调节膀胱功能机制研究进展

排尿功能障碍性疾病是临床常见病,其发病率逐年升高并呈现出年轻化趋势,针刺在临床中治疗该类疾病获得较好疗效,且目前已开展一系列的作用机制研究。本文通过文献检索近10年针刺对膀胱功能生理和病理调控的机制研究,分析总结针刺在神经递质表达与释放、逼尿肌自发收缩活性、传入神经兴奋性、细胞凋亡以及脑内神经元活性等方向的作用特点,发现针刺可通过多途径、多靶点的效应机制对膀胱功能起到良好调节作用; 但亦发现近年相关研究更侧重于对逼尿肌自发性收缩的调控,涉及高位中枢影响的研究较为空缺,而膀胱功能障碍性疾病的发病机制复杂,并非单一因素所致,因此建立多学科、多维度交叉的研究模式,将更有助于探索针刺的起效机制。

Research progress on the mechanism of acupuncture regulating bladder function

Voiding dysfunction is a common clinical disease,its incidence showed an increasing and a younger trend.Acupuncture has achieved excellent effect in treating this disease in clinical practice,and a series of mechanism research has been carried out.In this paper,we searched the mechanism of acupuncture on the physiological and pathological regulation of bladder function in recent 10 years,the characteristics of acupuncture in the expression and release of neurotransmitters,spontaneous contractile activity of detrusor,excitability of afferent nerve,apoptosis and activity of neurons in brain were analyzed and summarized.It is found that acupuncture play an important role in regulating bladder function through the mechanism of multi-channel and multi-target effect.However,it is also found that existing related studies have focused on the regulation of spontaneous contraction of detrusor,but lake of studies related to the higher nervous center.The pathogenesis of bladder dysfunction is complex,which is not caused by a single factor.Therefore,establishing a research model of multidisciplinary and multidimensional intersection will be more helpful to explore the effective mechanism of acupuncture.
