食管癌患者生命意义的现状及影响因素分析*

目的: 调查食管癌患者生命意义及自尊水平现状,分析生命意义与自尊的相关性,了解食管癌患者生命意义的影响因素。方法: 采用方便抽样法选取于天津市某三级甲等医院治疗的212例食管癌患者为研究对象,使用一般资料调查表、生命意义量表及自尊量表进行问卷调查。结果: 食管癌患者生命意义总分为（75.72±8.60）分,总体处于中等偏低水平;自尊总分为（25.05±3.23）分,处于低自尊水平;生命意义总分及各维度与自尊总分均成正相关（r=0.435~0.617,P<0.01）;多元逐步回归分析显示,自尊、文化程度及婚姻状况对生命意义均有影响（P<0.01）。结论: 食管癌患者生命意义感及自尊水平均有待提高,医护人员应指导患者采取积极的生活态度,与患者进行有效沟通,提供针对性个体化的干预措施及心理指导,以提升患者的自尊水平及生命意义感。

Study on the current status and influencing factors of meaning in life in patients with esophageal cancer

Objective: To investigate the status of meaning in life and self-esteem in esophageal cancer patients, to explore the correlation between them, and to understand the influencing factors of meaning in life in the patients. Methods: 212 patients with esophageal cancer in a tertiary Class A hospital in Tianjin were selected by the convenient sampling method. Questionnaires were surveyed using the general data questionnaire,Chinese version of meaning in life scale and self-esteem scale. Results: The total score of meaning in life of patients with esophageal cancer was 75.72±8.60, which was generally at a low level. The score of self-esteem was 25.05±3.23, which was at a low level. The scores of meaning in life and each dimension were positively correlated with the score of self-esteem(r=0.435~0.617, P<0.01).Multiple stepwise regression analysis showed that self-esteem, education, and marital status affected the meaning in life(P<0.01). Conclusion: Patients with esophageal cancer need to improve their sense of meaning in life and self-esteem. Medical staff should guide patients to adopt a positive attitude towards life, communicate effectively with patients, and provide targeted individualized interventions and psychological guidance to improve patients' self-esteem and sense of meaning in life.
