粉尘螨滴剂联合孟鲁司特钠片治疗小儿变应性鼻炎的疗效观察

目的:探讨粉尘螨滴剂联合孟鲁司特钠片治疗小儿变应性鼻炎(allergic rhinitis,AR)的临床疗效。方法:选取2016年1月至2018年8月中国人民解放军第一七四医院收治的AR患儿238例,根据随机数字表法分为对照组和研究组,每组119例。对照组患儿给予孟鲁司特钠片治疗,研究组患儿在对照组的基础上联合粉尘螨滴剂治疗。观察两组患儿的临床疗效,治疗前后的症状评分,白细胞介素4(IL-4)、白细胞介素6(IL-6)、白细胞介素17(IL-17)、免疫球蛋白E(IgE)及免疫相关指标水平变化情况,记录两组患儿治疗期间不良反应发生情况。结果:研究组患儿的总有效率为91.60%(109/119),明显高于对照组的71.43%(85/119),差异有统计学意义(P<0.05)。治疗后,两组患儿流涕、喷嚏、鼻塞及鼻痒症状评分与治疗前相比均明显降低,且研究组患儿上述评分明显低于对照组,差异均有统计学意义(P<0.05)。治疗后,两组患儿IL-4、IL-6、IL-17及IgE水平与治疗前相比均明显降低,且研究组患儿上述细胞因子水平明显低于对照组,差异均有统计学意义(P<0.05)。治疗后,两组患儿CD3+、CD4+及CD4+/CD8+水平与治疗前相比均明显升高,且研究组患儿上述指标水平明显高于对照组;两组患儿CD8+水平与治疗前相比明显降低,且研究组患儿CD8+水平明显低于对照组,差异均有统计学意义(P<0.05)。研究组、对照组患儿不良反应发生率分别为12.61%(15/119)、10.08%(12/119),两组的差异无统计学意义(P>0.05)。结论:粉尘螨滴剂联合孟鲁司特钠片治疗小儿AR的疗效确切,可有效改善患儿机体炎症因子水平,提高免疫功能,且不增加不良反应。

Efficacy of Dermatophagoides Farinae Drops Combined with Montelukast Sodium Tablets in the Treatment of Allergic Rhinitis in Children

OBJECTIVE:To probe into the clinical efficacy of dermatophagoides farinae drops combined with Montelukast sodium tablets in the treatment of allergic rhinitis(AR)in children.METHODS:Totally 238 children with AR admitted into 174 Hospital of the People’s Liberation Army from Jan.2016 to Aug.2018 were extracted to be divided into the control group and the study group via the random number table,with 119 cases in each group.The control group was given Montelukast sodium tablets,while the study group additionally received dermatophagoides farinae drops based on the control group.The clinical efficacy,changes in symptom scores,interleukin-4(IL-4),interleukin-6(IL-6),interleukin-17(IL-17),immunoglobulin E(IgE),and immune-related indicators before and after treatment were observed,and the incidence of adverse drug reactions of two groups were recorded.RESULTS:The total effective rate of the study group was 91.60%(109/119),significantly higher than 71.43%(85/119)of the control group,the difference was statistically significant(P<0.05).After treatment,the symptom scores of runny nose,sneezing,nasal congestion and nasal itching in two groups were significantly lower than those before treatment,and the study group was signficantly lower than the control group,the difference was statistically significant(P<0.05).After treatment,the levels of IL-4,IL-6,IL-17 and IgE in two groups were significantly lower than those before treatment,and the study group was significantly lower than the control group,the difference was statistically significant(P<0.05).After treatment,the levels of CD3+,CD4+and CD4+/CD8+in two groups were significantly higher than those before treatment,and the study group was significantly higher than the control group;the levels of CD8+in two groups were significantly lower than those before treatment,and the study group was significantly lower than the control group,with statistically significant difference(P<0.05).The incidences of adverse drug reactions in the study group and the control group were respectively 12.61%(15/119)and 10.08%(12/119),the difference was not statistically significant(P>0.05).CONCLUSIONS:The efficacy of dermatophagoides farinae drops combined with Montelukast sodium tablets in the treatment of AR is significant,which can effectively improve the level of inflammatory factors in children and improve immune function without increasing the incidence of adverse drug reactions.
