改良Sugiura术与Hassab术治疗食管静脉曲张的对比研究

目的研究对比改良Sugiura术与Hassab术治疗乙肝肝硬化相关性门脉高压症合并食管静脉曲张的效果。方法回顾性分析2011年1月~2012年7月在我科行改良Sugiura术或Hassab术的满足研究条件的两组患者的相关资料,对手术时间、出血量、并发症、术后食管静脉曲张的改变情况和再出血率等进行对比研究和统计分析。结果改良Sugiura术式组与Hassab术式组比较,术中出血量有增加(P0.001)、手术时间延长(P0.001),差异有统计学意义,但术后严重并发症的差异无统计学意义(P0.05);改良Sugiura术式组术后食管静脉曲张改善率优于Hassab术式组(P0.001),根除率高于Hassab术式组(P0.001),再出血率均低于Hassab术式组(P0.001),差异均有统计学意义。结论改良Sugiura术治疗食管静脉曲张的彻底性优于Hassab术,有利于控制再出血,改善远期预后。

Comparative Study of the Modified Surgiura's Operation with the Hassab's Operation for treating Esophageal Varices

Objective To study the different curative effect between the modified Surgiura＇ s operation and the Hassab＇ s oper- ation for esophageal varices which were caused by portal hypertension of liver cirrhosis of hepatitis B. Method The clinical data, in- eluding operating time, amount of bleeding, complication, change of esophageal varices, rebleeding rate, ere, of 81 patients who met the investigative condition from 2011 -01 to 2012 -07 in our department, were analyzed retrospectively. Result Compared with the Hassab＇ s operation, the modified Surgiura＇ s operation got longer operating time and larger amount of bleeding （P 〈 0. 001 ）, but got the same level in serious complieation（P 〉0.05）. However, the rate of improvement and eradication of the esophageal varices were both obviously higher in the modified Surgiura＇ s operation than the Hassab＇ s operation （P 〈 0. 001 ）, and the rate of relapse and re- bleeding were lower in the former（ P 〈 0. 001 ）. Conclusion The modified Surgiura＇ s operation was more effective than the Hassab＇ s operation in curing esophageal vaxices, and it could also reduce the rate of rebleeding, and improve the long-term prognosis.
