rhGM-CSF及rb-bFGF治疗儿童小面积浅度烧伤创面临床对比研究

目的：了解外用重组人粒细胞巨噬细胞集落刺激因子（rhGM-CSF）凝胶剂（金扶宁）及外用重组牛碱性成纤维细胞生长因子（rb-bFGF）（贝复济）治疗儿童Ⅱ度烧伤创面的有效性及安全性。方法：将2012年6月至2012年11月来我院就诊的小面积浅度烧伤患儿45例按抽签法随机分为三组,分别为金扶宁治疗组20例、贝复济治疗组20例、生理盐水对照组5例。金扶宁组予rhGM-CSF 0.5μg/cm2直接涂于创面,贝复济组给予rb-bFGF 0.03 mL/cm2直接均匀喷于创面,生理盐水组给予生理盐水0.03 mL/cm2直接均匀喷于创面,比较治疗组与对照组创面愈合时间。结果：创面愈合时间金扶宁组（8.619±0.944）d、贝复济组（10.766±1.369）d较对照组（13.850±0.398）d明显缩短,差异有统计学意义（P〈0.05）,用药期间患儿血常规及肝肾功相关指标的差异与药物应用无明显相关。结论：金扶宁及贝复济处理小面积浅Ⅱ度烧伤均较生理盐水能有效缩短愈合时间,加快愈合速度,对于儿童生长发育无明显影响,未发现明显不良反应,在儿童创面的治疗应用中安全、有效。

Clinical Research on rhGM-CSF and rb-bFGF in Treating Small Shallow Burn Wound in Children

Objective： This study is aimed to evaluate the efficacy and safety of rhGM-CSF and rb-bFGF on wound healing of burns in children. Methods： In our hospital from June 2012 to November 2012, 45 cases with a small area of shallow bums were randomly divided into three groups by balloting method： the rhGM-CSF treatment group of 20 patients, the rb-bFGF treatment group of 20 cases, and the saline control group of five cases. The rhGM-CSF treatment group received rhGM-CSF 0.5 μg per square centimeter, which were painted directly on the wound. The rb-bFGF treatment group received rb-bFGF 0.03 mL per square centimeter. The saline control group received saline. The healing time of each group was compared. Results： Wound healing time in the two treatment groups （8. 619±0. 944 days vs 10. 766±1. 369 days） were shorter than that in the control group （ 13. 850±0. 398 days） ; there was a significant difference （ P〈0.05 ）. No significant correlation was observed between the differences of blood count and liver and kidney function indexes and drug application in children during treatment. Conclusions： rhGM-CSF and rb-bFGF can significantly accelerate wound healing in children, and no adverse reaction was observed in the study.
