共同动脉干11例影像学诊断分析

摘    要:目的 探讨X线平片、二维超声心动图(2D-UCG)、MRI及心血管造影在先天性心脏病共同动脉干诊断中综合应用的价值.方法 回顾分析1990年1月至2010年3月经造影或/和手术证实的共同动脉干患儿11例的影像学检查资料,所有患儿均行X线平片、2D-UCG及心血管造影检查.结果 X线平片显示7例(64%)患儿肺血增多,所有患儿心脏增大;2D-UCG检查能准确清楚地显示心腔内结构,如瓣膜、瓣环、房室间隔,与心血管造影或/和手术符合率达到64%(7/11);心血管造影能全面地显示心腔内外结构,患儿均明确诊断.结论 共同动脉干的影像学检查技术各有优劣,其中以心血管造影检查最为准确.

Abstract:Objective To explore the comprehensive application value of the X-ray plain film, 2-dimensional echocardio-graphy (2D -UCG), MRI and cardiovascular angiography for the congenital heart disease common artery. Methods A retro-spective analysis was conducted on 11 patients of truncus arteriosus referred for surgical operation treatment. X-ray films, 2EP UCG and angiocardiography were underwent. Results Seven patients (64%) with pulmonary blood increase were showed by X-ray plain film, and all children with cardiac enlargement; the heart cavity structure was clearly displayed by 2D-UCG examination, such as valve, central flap, atrio-ventricular septal, and reached coincidence rate of 64% (7/11) with cardiovascular angiography and/or surgical result~ the heart cavity internal and external structure was fully displayed by angiocardiography, and to make a definitive diagnosis. Conclusion The imaging diagnosis of truncus arteriosus, all imaging technology have their advantage and weakness. Angiography is the most accurate method.
