Solitaire AB支架辅助弹簧圈栓塞颅内宽颈动脉瘤33例疗效

目的 研究Solitaire AB支架辅助弹簧圈栓塞颅内宽颈动脉瘤的可行性、安全性及疗效.方法回顾2010年1月至2011年12月期间采用Solitaire AB支架辅助弹簧圈栓塞33例颅内宽颈动脉瘤,通过动脉瘤栓塞后的形态及患者临床状态来分析治疗效果.结果 33例患者支架位置放置准确,均完全覆盖瘤颈.27例GOS（Glasgow Outcome Score 格拉斯哥预后评分）5分,2例GOS 4分,1例GOS 3分,1例GOS 1分.平均临床随访时间10个月,2例未随访.血管造影结果显示25例彻底栓塞,3例残留部分瘤颈,2例不完全栓塞.所有动脉瘤均未再出血.结论 Solitaire AB支架操作容易,放置准确,辅助弹簧圈栓塞颅内宽颈动脉瘤安全、有效,临床效果满意.

Observation of the effect for Solitaire AB stent-assisted coil embolisation of wide-necked intracranial aneurysms

Objective To research the feasibility, security and curative effect of Solitaire AB stent-assisted coil embolisation of wide-necked intracranial aneurysms. Methods From 2010 to 2011, 33 patients harboring wide-necked intracranial aneu- rysms were treated with Solitaire AB stent placement and aneurysm coiling. We assessed the clinical history, aneurysm dimen sions, and technical details of the procedures, including any difficulties with stent deployment, degree of aneurysm occlusion, and complications, and evaluated the clinical results by means of GOS （Glasgow outcome score）. Results Successful deployment of the stent in the target artery was achieved in all patients. Majority of the patients had good outcomes. GOS was 5 in 27 patients, 4 in 2 patients, 3 in 1 patient and 1 in 1 patient. Angiogram showed complete occlusion in 25 patients, nearly complete in 3 patients, and incomplete was present in 2 patients. Conclusion Solitaire AB stents are operated easily, and placed accurately; Solitaire stent-assisted coil to embolize wide-necked intracranial aneurysms is safe and effective; the clinical result is sat isfactory.
