超声引导在婴儿股静脉穿刺置管中的应用

目的 评价超声引导下行婴儿股静脉穿刺置管的效果.方法选择患儿40例,年龄3 d～12个月,体质量2.1～8.9 kg.随机分为两组,解剖标志定位组（A组,n=20）根据体表解剖标志定位行股静脉穿刺置管,超声引导组（U组,n=20）在超声引导下行股静脉穿刺置管.记录穿刺时间、穿刺成功率和并发症的发生情况.结果 A组成功置管17例,U组全部成功置管.与A组比较,U组穿刺时间较短（P〈0.05）,且首次穿刺成功率也高于A组（P〈0.05）.A组2例误穿股动脉,3例穿刺部位血肿;而U组无一例出现并发症.结论 超声引导用于婴儿股静脉穿刺置管成功率高,穿刺时间短,可避免多次穿刺且并发症少.

Application of ultrasound-guided cannulation of the femoral vein in infants

Objective To evaluate the efficacy of ultrasound-guided cannulation of the femoral vein in infants. Methods Forty infants aged 3 days-12 months weighing 2.1-8.9 kg were randomly divided into two groups; one group in which the femoral vein cannulation was guided by anatomical landmarks （group A, n=20）, and the other group in which the US-guided technique was used for cannulation （group U, n=20）. The time spent performing the cannulation, successful puncture and compli- cations were recorded. Results The procedure was successful in 17 cases of group A, while in all cases of group U. The time spent performing the cannulation was significantly shorter in group U than that in group A. The number of first-pass success was more in group U. Two cases of femoral arterial puncture and three cases of local hematoma occurred in group A, while there were no complications in group U. Conclusion The US-guided technique for femoral vein cannulation in infants is useful as it results in greater success, shorter cannulation times, fewer attempts, and fewer complications.
