甘露聚糖肽联合顺铂治疗恶性胸水的临床疗效观察

目的探讨甘露聚糖肽联合顺铂治疗恶性胸水的临床疗效。方法收集2011年12月～2012年12月在我院就诊的恶性胸水70例，随机分为观察组和对照组各35例，两组均行常规胸腔穿刺闭式引流术，观察组同时加甘露聚糖肽联合顺铂治疗，对照组同时加顺铂治疗，比较两组的疗效及两组生活质量改善情况。结果观察组治疗后的总有效率达82．9％。其疗效明显优于对照组，差异具有显著性（x^2=4．738，P〈0.05）。观察组治疗后的生活质量改善状况明显优于对照组，差异具有显著性（u=3．65，P〈0.05）。对照组疗效评价为进展的2例患者中，1例为贲门癌晚期同时伴胸、腹腔积液，1例出现引流管堵塞，用生理盐水冲管后通畅；观察组6例稳定期患者，其中3例注药后出现发热、胸痛、恶心，经对症治疗后缓解。两组均未继发胸腔内感染，无肝肾功能损害。结论甘露聚糖肽联合顺铂治疗恶性胸水的疗效确切，可以明显改善患者的生活质量，值得推广和应用。

Mannatide cisplatin in the treatment of malignant pleural effusion observation

Objective To investigate the effect of Mannatide and cisplatin treatment of malignant pleura1 effusion. Methods From December 2011 to December 2012 in our hospital,70 cases of malignant pleural effusion were randomly divided into observation group and control group, 35 patients in both groups underwent routine thoracentesis with closed drainage, while the observation group plus Mannatide cisplatin, while the control group plus cisplatin treatment, the efficacy and quality of life improvement were compared between two groups. Results After treatment, the total effective rate of the observation group was 82.9%, and its efficacy was significantly better than the control group, the difference was significant （x^2 = 4.738, P 〈0.05）. After treatment, the quality of life to improve the situa- tion of the observation group was significantly better than the control group, the difference was significant （u = 3.65, P 〈0.05）. Clinical evaluation of progress in the control group 2 patients, 1 patient with advanced gastric cardia was accompanied by pleural and peritoneal effusion, 1 case of drainage tube blockage, washed with saline tube patency; observation group of 6 patients with stable patients, three cases after injection, fever, chest pain, nausea, after symptomatic remission after treatment. The two groups were not secondary chest infection, liver and kidney dysfunction. Conclusion Mannatide and cisplatin in treatment of malignant pleural effusion have good effect, can significantly improve the patient＇s quality of life, worthy of promotion and application.
