高压氧治疗新生儿重症肺炎肺功能动态变化及疗效研究

摘    要:目的探讨高压氧治疗新生儿重症肺炎肺功能动态变化和疗效及安全性,为治疗新生儿重症肺炎提供依据。方法将新生儿重症肺炎89例随机分为高压氧组与对照组,两组均采用相同的治疗,抗感染、鼻导管或者头罩吸氧、止咳、强心、利尿及雾化等治疗,高压氧组在上述基础上采用YLCO.5/1.2型婴儿高压氧舱进行高压氧治疗,每次30min,1次,d,4d为一个疗程,氧舱压力0.20~0.25MPa,对肺功能进行动态监测,并对两组患儿出院后进行定期随访。结果高压氧组经治疗后各症状体征持续时间均短于对照组,且高压氧治疗后患儿肺功能明显改善.随着高压氧治疗次数增多肺功能改善更加明显,与对照组比较有显著性差异,两组治疗后均未见失明、视力异常、减压病等副作用。结论高压氧组治疗新生儿重症肺炎疗效明显优于对照组,尤其是对新生儿重症肺炎肺功能改善明显,且未见明显副作用。提示高压氧可作为治疗新生儿重症肺炎较理想的方法。

The study on dynamic change of pulmonary function and curative effect of hyperbaric oxygen in the treatment of neonatal severe pneumonia

Abstract:Objective To observe the curative effect and side effects of hyperbaric oxygen in the treatment of severe pneumonia in neonates, in order to seek the new method and the proof of treat the neonatal severe pneumonia. Meth- ods A total of 89 patients were randomly divided into the hyperbaric oxygen group and the control group, they were treated by the resistance to infection, nasal catheter and head cover oxygen, cough, strong heart, diuresis and atomiza- tion and so on. In addition, the patients of hyperbaric oxygen group were treated by hyperbaric oxygen with YLC0.5/ 1.2 baby hyperbaric oxygen chamber. Every time 30 minutes, 1 time/day, 4 days for a period of treatment and the oxy- gen tank pressure was 0.20 - 0.25 Mpa, to the dynamic monitoring of pulmonary function, and the two groups of chil- dren with regular follow-up after discharge. The patients of two groups were determined by a follow-up telephone call after discharge. Results There was significant difference between two groups. The symptoms and signs, compared to these before treatment of the two groups both got better obviously. Two groups were not seen obvious side effects. Pul- monary function of hyperbaric oxygen group improved gradually. Conclusion The clinical symptoms were improved in both groups, while the effective treatment in hyperbaric oxygen group is better than control group, and has not seen the obvious untoward effect occurrence.
