妊娠期糖尿病发病情况及其影响因素分析

目的探讨我区孕妇妊娠期糖尿病发病情况和影响因素。方法对2010年1月－2012年12月期间在我院进行产前保健的912例孕妇进行糖尿病筛查．将筛查出的GDM患者作为研究组，选择110例正常孕妇作为对照组，对两组孕妇进行GDM危险因素分析。结果所有筛查对象中GDM阳性率为13．7％（125／912）；BMI、高Hb血症、高脂血症、家族糖尿病史和不良孕产史与GDM发病相关联，而澳抗阳性和空腹尿糖阳性与GDM发病不存在关联；多因素Logistic回归分析显示BMI、不良孕产史、糖尿病家族史和高Hb血症是GDM发病的独立危险因素。结论我区女性妊娠期糖尿病的发病与孕前体重超重、糖尿病家族史、不良孕产史和高Hb血症有密切关系，对具有以上四种危险因素的孕妇，应进行糖尿病筛查。

Analysis of morbidity and influence factors of gestational diabetes mellitus

Objective To investigate the morbidity and influence factors of gestational diabetes mellitus of pregnant women in our district. Methods A screening for gestational diabetes mellitus （GDM） was conducted in 912 pregnant women with the prenatal care during January 2010 to December 2012 in our hospital, the GDM patients were as the research group, 110 normal pregnant women were as the control group, analyzed the risk factors of GDM of the pregnant women in two groups. Results The positive rate of GDM for screening of all objects was 13.7% （125/912）; BMI, High hemoglobin, hyperlipidemia, family history of diabetes and adverse pregnancy outcomes were associated with GDM, Australia antigen and fasting glucose were not associated with GDM; Multivariate Logistic regression analysis showed that BMI, adverse pregnancy history, family history of diabetes and high serum Hb was an independent risk factor in the pathogenesis of GDM. Conclusion The women of gestational diabetes incidence is closely related to pre-pregnancy overweight, family history of diabetes, history of abnormal pregnancy and High hemoglobin, pregnant women with four risk factors above should take diabetes screening.
