| 摘 要： | 目的探讨艾司西酞普兰联合心理干预对糖尿病患者抑郁焦虑症状及血糖达标的影响。方法72例糖尿病伴抑郁焦虑患者随机分为观察组和对照组。两组均予以口服降糖药或皮下注射胰岛素控制血糖。观察组加用艾司西酞普兰联合心理干预治疗，对照组加用艾司西酞普兰治疗，疗程均为8周。结果治疗8周后，两组患者HAMD和HAMA评分均较前明显改善（P〈0．05或P〈0．01），且观察组改善幅度更明显（P〈0．05）；同时观察组患者空腹血糖、餐后2h血糖和糖化血红蛋白完全达标率均明显高于对照组（P〈0．05或P〈0．01）．观察组低血糖事件发生率明显低于对照组（x^2=5．73，P〈0．05）。结论艾司西酞普兰联合心理干预能有效缓解糖尿病患者的抑郁和焦虑症状，有利于患者血糖的达标，减少糖尿病相关并发症的发生。

| Abstract: | Objective To discuss the influence of Escitalopram combined with mental intervention on symptoms of de- pression and anxiety and compliance rate of blood sugar of diabetics. Methods A total of 72 cases of diabetics with depression and anxiety were divided into observation group and control group at random. The patients in two groups were given hypoglycemic drugs through the mouth or insulin by hypodermic injection to control the blood sugar. Additionally, the patients in observation group were given Escitalopram combined with mental intervention, and the patients in control group were given Escitalopram for 8 weeks＇ course of treatment. Results After 8 weeks＇ medical treatment, the HAMD and HAMA scores of patients in two groups were improved obviously than before （P〈0.05 or P〈0.01）, and the improved rate in observation group was much higher than that in control group （P〈0.05）. Meanwhile, total compliance rates of fasting blood-glucose, blood sugar and glycosylated hemoglobin 2 hours after dinner of patients in observation group were much higher than those in control group （P〈0.05 or P〈0.01） .The occurrence rate of glucopenia cases were appeared in observation group was much lower than that in control group （x^2=5.73 ,P〈0.05）. Conclusion Escitalopram combined with mental intervention has reliable effect on symptoms of depression and anxiety, the compliance rate of blood sugar of diabetics, the reduction of the occurrence rate of glucopenia cases and relevant complication, and the improvement of prognosis of diabetics. |
