甲氧明泵注对腰硬联合麻醉下剖宫产血流动力学的影响

摘    要:目的观察剖宫产术麻醉后甲氧明微量泵变速输注产妇血流动力学的变化。方法将40例产妇随机分为甲氧明组(A组)和麻黄碱组(B组),每组20例。甲氧明组腰麻后开始变速微量泵输注,输注过程中酌情加用麻黄碱:麻黄碱组当患者麻醉成功平卧后静注麻黄碱,维持产妇收缩压100mmHg以上。结果麻醉后两组DBP在T4时间点上差异有显著性(P〈0.05)。两组HR在T2、T3、T4、T5时间点上差异有显著性(P〈0.01),F=3.943,P=0.006〈0.01,具有统计学意义。结论甲氧明微量泵变速输注联合小剂量麻黄碱用于处理产科麻醉中的低血胝可以提供更加稳定的血流动力学。

Abstract:Objective To compare hemodynamics differences between methoxamine, combined with ephedrine neces- sarily,infused by micro pump and ephedrine only in the anesthesia of cesarean section. Methods Forty patients were randomly divided into two groups:methoxamine group (group A) and ephedrine group (groupB), 20 cases of each group. Group A :methoxamine infusion by micro pump after combined spinal-epidural injection. Group B:ephedrine intravenously injected after the patients anesthetized. Maintain systolic blood pressure above 100 mmHg. Results DBP had significant difference in the T4 time point(P〈0.05), HR of two groups had significant difference in T2, T3, T4,T5 time point(P〈0.01 ), F=3.943, P=0.006(P〈0.01 ) with statistical significance. Conclusion Methoxamine combined with small dose of ephedrine necessarily infused by micro pump in the anesthesia of cesarean section for treatment of hypotension can provide more stable hemodynamics.
