摘    要:①目的 探讨钛网融合器在胸腰椎不稳定性爆裂性骨折中的应用效果。②方法 20例病人应用钛网融合器加肋骨支撑移植治疗。所有病人手术中彻底切除向椎管内挤压的碎骨片,完全减压。③结果 无病人在减压和稳定治疗后神经症状加重,无病人出现医源性神经损伤及血管损伤,无病人出现内固定松动及骨移植失败现象。④结论 前路手术加钛网融合器在椎管清除率、术后载荷效果、降低骨移植失败方面疗效明显,对神经恢复能创造最适宜的条件。

Abstract:Objective To investigate the therapeutic effect of titanium mesh cage in the treatment of unstable burst fractures of the thoracolumbar spine. Methods Twenty patients underwent implantation with iliac crest and ribs as strut graft of titanium mesh cage internal fixation. The retropulsed fragments were completely removed in all the cases for complete decompression. Results The patients were stable and no neurological deterioration was noted after the anterior decompression . No iatrogenic neurological or vascular injury occurred. No patients suffered from looseness or failure of the implants. No implants needed to be removed. Conclusion Anterior procedures and titanium mesh cage internal fixation are superior to posterior procedures in terms of canal clearance, effective load-sharing, reduced implant failure and restoration of the nerves.
