彩色多普勒超声对胎儿心脏畸形的诊断价值

目的探讨彩色多普勒超声诊断胎儿心脏畸形的临床价值。方法采用彩色多普勒超声对148例孕龄20周。41周的胎儿心脏行超声检查，研究正常胎儿心脏各切面的扫查方法、胎儿心脏各腔室测值与孕龄的关系，观察有无先天性畸形，并作跟踪随访。结果胎儿心脏各腔室大小与孕龄成正相关关系，正常胎儿心胸比例1：3左右，为右心优势。148例中检出三尖瓣下移畸形1例，完全性心内膜垫缺损1例，法乐氏四联症1例，大血管转位1例，室间隔缺损4例，其中1例合并胎儿淋巴管瘤畸形，卵圆孔直径增大(大于6mm)4例，心肌肥厚1例，心律失常17例。结论彩色多普勒超声是诊断胎儿先天性心脏畸形的首选方法，具有无创、安全、准确的优点。

Diagnostic value of color Doppler ultrasound for fetal cardiac malformation

Objective To assess the value of color Doppler ultrasound in the diagnosis of fetal cardiac malformation.Methods A hundred and forty-eight fetals with gestational age of 20 to 41 weeks underwent color Doppler ultrasonography of cardiac all sections and the correlation between cardiac cavities-ventriculars and gestational age were studied,simultaneously,the presence or lack of congenital cardiac malformation were observed and followed-up.Results The measurement value of all cavities and ventricules of fetal heart were found to be directly proportional to gestational age,normal ratio between heart and thorax of fetus was approximately 1:3,right heart was predominant.Among 148 cases,color Doppler ultrasonography demonstrated the deformity of tricuspid valve downward movement in 1,complete endocardium pad defect in 1,tetralogy of Fallot in 1,great vessels shift in 1,ventricular septal defect in 4,Botallo's foramen enlargement (> 6 mm)in 4,myocardial hypertrophy in 1.and arrhythmia cordis in 17.Conclusion Because color Doppler ultrasound in the detecting of fetal heart is untraumatic,safe and accurate,it should be first selective means.
