巩膜环扎术预防穿孔性眼外伤后视网膜脱离

在４５例累及后段的穿孔性眼外伤病例行玻璃体切割术中，随机对其中２５例同时行预防性巩膜环扎术，观察环扎术对后期视网膜脱离发生率的影响。环扎术采用３ｍｍ宽的硅胶带置于角巩缘后９ｍｍ至１２ｍｍ之间，形成一个中等高度的巩膜嵴。经６个月以上的随访，行环扎术的２５例中发生视网膜脱离５例（２０％）；未行环扎术的２０例中有７例发生视网膜脱离（３５％）（Ｐ＜０．０５）。本结果显示：预防性巩膜环扎对穿孔性眼外伤后期视网膜脱离发生率的降低是有益的。

Prophylactic scleral buckling for delayed retinal detachment after perforating ocular injuries

To study the role of prophylactic scleral buckling in delayed retinal detachment after perforating ocular injuries involving the posterior segment,fouty five patients undergoing vitrectomy were randomly assigned to either conventional management or in addition to receive a encircling scleral buckling procedure.The prophylactic encircling scleral buckle consisted of a 3mm silicone band sutured to the 9 12mm from the limbus and tightened to achieve a moderate buckling efect.All patients were followed for more than 6 months.Five of twenty five patients(20%)who undergo scleral buckling at the time of the vitrectomy developed delayed retinal detachment.While in twenty patients without scleral buckling development of this complication was in seven(35%)(p<0 05).This finding supports that prophylactic scleral buckling is benificial in the treatment of eyes with perforating injuries involving the posterior segment to prevent delayed retinal detachment.
