| 摘 要： | 目的探讨经小隐静脉(SSV)插管予以阿加曲班和尿激酶区域抗凝溶栓治疗急性下肢深静脉血栓(LDVT)的临床价值。方法前瞻性地将56例急性LDVT随机分为两组,研究组(21例/24条肢体)予以经SSV插管予以阿加曲班和尿激酶区域抗凝溶栓,对照组(35例/36条肢体)经外周静脉予以阿加曲班和尿激酶,分析两组血清纤维蛋白原(FBG)和D-二聚体变化,调查静脉血栓的消长、患肢周径及溶栓抗凝相关并发症。结果经校正χ2检验,研究组相关并发症发生率低于对照组(1/21 vs 4/36,χ2=1.92,P≤0.05)。经威尔科克森检验,研究组总有效率(95.8%,23/24)和对照组(94.4%,34/36)无统计学差异(V=0.52,P>0.05),但总显效率存在统计学差异(83.3%vs 55.6%,V=2.36,P≤0.05)。经配对t检验,研究组溶栓期间血清FBG无明显变化(P>0.05)。而对照组血清FBG明显下降(P≤0.05)。研究组患肢周径减少时相和血清D-二聚体下降时相早于对照组(P≤0.05)。结论经SSV插管予以区域灌注阿加曲班和尿激酶可提高溶栓疗效,减少出血风险。
