从传统辨证方法入手探讨传染性非典型肺炎辨证

传染性非典型肺炎(严重急性呼吸综合征,SARS)属中医温疫病范畴,其传染性强、发病快、变化多,与以往一般温病,如风温、春温不同,与过去的伤寒也不同.有必要对SARS辨证方法进行探讨,从辨证的科学思维出发,研究建立SARS的辨证模式.卫气营血辨证和三焦辨证都是用以分析温病病理变化,确定病变部位,揣度病势轻重,认知病情轻重和归纳证候类型的方法.两者在很大程度上有共通之处,临床把两者有机结合,结合六经辨证,才能全面指导温病的辨证论治.根据SARS临床表现确定中医"证素",结合病因病机及传统六经辨证、卫气营血及三焦辨证,可归纳SARS的分期及证候演变规律.

Study on Differentiation of Syndromes of SARS the Method of Traditional Syndrome Differentiation

Infectious atypical pneumonia (severe acute respiratory syndrome, SARS) belongs to the category of infectious epidemic disease in TCM characterized by strong infectivity, rapid attacking, variableness, which is different to past common epidemic febrile disease such as wind-warm and spring-warm syndromes and to the past exogenous febrile disease. It is necessary to probe differentiation methods of syndromes of SARS and establish SARS syndrome differentiation model. Both the syndrome differentiation of We, Qi, Ying and blood and the syndrome differentiation of tri-Jiao are the methods for analyzing pathological changes of epidemic febrile diseases, determining the location of lesion, conjecturing the severity of disease, understanding the state of illness and inducing types of syndromes. The both methods have something in common to a considerable degree. Clinically, organic combination of the both methods in combination with syndrome of differentiation of six meridians can overall guide treatment of epidemic febrile diseases based on syndrome differentiation. Based on clinical manifestations of SARS,to determine "syndrome elements" of TCM and to combine with pathogenic factors and pathogenesis, and the traditional syndrome differentiation of six meridians, the syndrome differentiation of Wei, Qi, Ying, blood and tri-Jiao, can induce staging and developing law of SARS.
