64层螺旋CT血管成像对脊髓血管畸形的初步研究

摘    要:目的通过与DSA对比,初步探讨64层螺旋CT血管成像(CTA)在诊断脊髓血管畸形方面的应用价值。方法6例患者中,男、女各3例。临床症状主要表现为下肢无力,查体主要见下肢肌力减退及感觉障碍。6例均行CTA和DSA检查。结果6例脊髓血管畸形患者分别为髓内动静脉畸形(2例)、髓周动静脉瘘(2例)、Cobb综合征(2例)。6例患者各部位畸形血管团、瘘口位置、供血动脉、增粗纡曲的引流静脉于CTA均可显示,与DSA所见基本一致。CTA在平均扫描时间15秒内即可完成检查,经各种重建方法可全面立体观察畸形血管与邻近结构的关系。结论64层螺旋CTA可以快速、无创、全面显示各种脊髓血管畸形的病变,缩短DSA诊疗时间,在脊髓血管畸形的诊断和协助制订治疗方案方面起到重要作用。

Abstract:Objective To study the clinical application of 64-slice CT angiography (CTA) in spinal vascular malformations by comparing with digital subtraction angiography (DSA). Methods Six patients (3 males and 3 females) had suffered from weakness or paralysis of lower limbs. Clinical examination revealed decline of muscle strength of lower extremities and a variety of sensory abnornalities. They underwent CTA and DSA. The findings of CTA were compared with those of DSA. Results Six patients were diagnosed as intramedullary arteriovenous malformation, extramedullary arteriovenous fistula, and Cobb syndrome, two cases respectively. Different types of spinal vascular malformations were all shown by CTA, and the location of fistula, feeding arteries, and engorged draining veins of the lesions were all detected by CTA. The findings in CTA correlated well with DSA. Furthermore, the examinations only need 15 seconds. The relationships between the lesions and surrounding structures were displayed completely and clearly using CTA with various reconstruction techniques. Conclusion 64-slice CTA can reveal the lesions in spinal vascular malformations quickly, harmlessly and entirely, as well as reduce the time consumption for DSA. It plays an important role in the diagnosis of spinal vascular malformations and helps to decide the remedy plan.
