炔雌醇环丙孕酮联合中药调周疗法治疗多囊卵巢综合征不孕症40例临床观察

目的观察炔雌醇环丙孕酮联合中药调周疗法治疗多囊卵巢综合征不孕症的临床疗效。方法将80例多囊卵巢综合征不孕症患者随机分为2组。对照组40例单纯于月经周期第5 d开始口服炔雌醇环丙孕酮,连续服用21 d,停药7 d后重复用药,连续应用3个月经周期,后于第4个月经周期第5 d开始予枸橼酸氯米芬片口服治疗5 d,连续治疗3个月经周期。治疗组40例在对照组炔雌醇环丙孕酮连续治疗3个月经周期后,再予中药人工周期治疗3个月经周期。观察2组治疗前后中医证候积分变化,治疗后的排卵情况,用B超监测患者治疗后优势卵泡个数、子宫内膜厚度及排卵天数,计算妊娠的成功率。结果对照组妊娠率32.5%,治疗组妊娠率57.5%,治疗组妊娠率高于对照组(P0.05);2组治疗后中医证候积分均改善(P0.05),且治疗组改善优于对照组(P0.05);治疗组治疗后优势卵泡个数、子宫内膜厚度及排卵天数均优于对照组(P0.05)。结论炔雌醇环丙孕酮联合中药调周疗法能有效缓解多囊卵巢综合征患者的临床症状,增加优势卵泡的个数,提高卵泡的质量,促进卵子排出,增加妊娠的成功率。

Clinical observation of ethinylestradiol-cyproterone acetate combined with menstruation regulation with traditional Chinese medicine on the treatment of polycystic ovary syndrome in infertile women

Objective To observe the clinical effect of ethinylestradiol-cyproterone acetate combined with menstruation regulation with traditional Chinese medicine on the treatment of polycystic ovary syndrome (POS) in infertile women.Methods 80 POS infertile women were randomly divided into two groups.40 subjects in control group were treated by oral ethinylestradiol-cyproterone acetate since days 5 of menstrual circle for 21 days,repeated dosage 7 d after withdrawal,continuous application for 3 menstrual cycle,and then subjects were orally taken with clomifene citrate tablets since days 5 of the forth menstrual circle for 5 days,continuously for three menstrual circles.40 subjects in treatment group were received ethinylestradiol-cyproterone acetate for three menstrual circles,and then were received traditional Chinese medicine (TCM) artificial cyclic therapy for three menstrual circle.The scores of TCM syndrome before and after treatment were observed in two groups.The amount of dominant follicle,the endometrial thickness and ovulation time were detected by B-ultrasound to calculate the successful pregnancy rates.Results The successful pregnancy rates in treatment group (57.5％) was superior to that in control group (32.5％,P ＜0.05).The improvement of TCM score in treatment group was more obvious than that in control group with statistical significance (P ＜ 0.05).The amount of dominant follicle,the endometrial thickness and ovulation time after treatment in treatment group were superior to those in control group (P ＜ 0.05).Conclusion Ethinylestradiol-cyproterone acetate combined with menstruation regulation with TCM can relief the symptoms of polycystic ovary syndrome,increase dominant follicle,improve ovulation,and enhance the successful pregnancy rates.
