依托咪酯对颈椎手术患者脊髓功能的影响

摘    要:目的:探讨依托咪酯对颈椎手术患者的脊髓保护作用。方法分析2012年10月至2014年10月行颈椎手术治疗的脊柱病变患者的资料,采用随机数表法将入选者随机分成观察组(依托咪酯组)和对照组(0儋.9%氯化钠溶液组)。比较2组患者术中生命体征,术前术后不同时刻炎性因子及脊髓功能相关指标。结果本研究共收集患者88例,其中观察组48例,对照组40例。 T2、T3时刻,观察组患者MAP、HR水平均显著高于对照组( P <0.05),而CVP则低于对照组( P <0.05);术后1 d、7 d时,观察组患者CRP、PG-E2、TNF-α和IL-6水平均显著低于对照组( P<0.05);术后1 d、7 d时观察组患者NSE、GFAP、MMP-9水平均显著低于对照组,而JOA评分高于对照组(P<0.05)。结论依托咪酯在颈椎手术中具有一定的脊髓保护功能。

Effects of etomidate on spinal cord function of patients receiving cervical spine surgery
Abstract:Objective To investigate the protective effects of etomidate on spinal cord of patients receiving cervical spine surgery.Methods The clinical data about 88 patients with spondylopathy who underwent cervical spine surgery in our hospital from October 2012 to October 2014 were retrospectively analyzed.According to random number table these patients were divided into two groups:observe group (etomidate, n =48) and control group (physiological saline, n =40).The vital signs during operation, inflammatory factors and the related indexes about spinal cord function in different time point before and after operation were observed and compared between two groups.Results The levels of MAP,HR in T2,T3 in observation group were significantly higher than those in control group ( P <0.05), however,the levels of CVP were significantly lower than those in control group ( P <0.05).The levels of CRP, PG-E2, TNF-αand IL-6 on 1d, 7d after operation in observation group were significantly lower than those in control group ( P <0.05).Moreover the levels of NSE, GFAP and MMP-9 on 1d, 7d after operation in observation group were significantly lower than those in control group,however, the JOA scores were significantly higher than those in control group ( P <0.05).Conclusion The etomidate has protective effects on spinal cord at certain extent during cervical spine surgery.
