肺鳞癌和腺癌中E-钙黏附素和基质金属蛋白酶-9的病理意义

摘    要:目的:检测肺癌组织中E-钙黏附素(E-cadherin)和基质金属蛋白酶-9( matrix metalloproteinase-9,MMP-9)的表达,并对两者与临床病理特征的关系进行了研究。方法应用免疫组织化学方法,对138例肺鳞癌和肺腺癌以及对应的癌旁正常肺组织中E-cadherin和MMP-9蛋白的表达情况进行了检测。结果肺癌组织中E-cadherin的阳性表达率为28.99%(40/138),正常肺组织为66.67%(92/138),两者比较,差异有统计学意义( P <0.05);在肺癌组织中,MMP-9的阳性表达率为59.42%(82/138),正常肺组织为29.71%(41/138),两者比较,差异有统计学意义(P<0.05);E-cadherin和MMP-9的表达水平均与肺癌的分化程度、TNM分期、淋巴结转移及术后生存时间有明显相关( P <0.05);两者的表达与病理类型、性别、年龄、吸烟史无关( P >0.05);E-cadherin蛋白的表达与MMP-9蛋白的表达呈负相关( P <0.05)。结论 E-cadherin低表达与MMP-9高表达是肺癌侵袭转移的重要生物学指标。

Expression and clinical significance of E-cadherin and MMP-9 in lung squamous carcinoma and adenocarcinoma
Abstract:Objective To observe the expressions of E-cadherin and matrix metalloproteinase-9 ( MMP-9 ) in lung cancer tissues,and to analyze the correlation between their expressions and clinicopathological characteristics of lung cancer . Methods The expression levels of E-cadherin and MMP-9 proteins were detected by immunohistochemistry ( SP ) in 138 cases of lung squamous carcinoma and lung adenocarcinoma as well as adjacent normal lung tissues .Results The positive expression rate of E-cadherin in lung cancer tissues was 28.99% (40/138),however,which was 66.67% (92/138) in normal lung tissues,there was a significant difference between them ( P <0.05).The positive expression rate of MMP-9 in lung cancer tissues was 59.42% (82/138),however,which was 29.71% (41/138) in normal lung tissues,there was a significant difference between them ( P <0.05).The expression levels of E-cadherin and MMP-9 were closely correlated to the differentiation degree ,TNM staging of lung cancer , lymph node metastasis of lung cancer and patient's survival time after surgery ( P <0.05), however, the expression levels of E-cadherin and MMP-9 were not related with pathological type of lung cancer,patient’s sex and age, smoking history ( P >0.05).Moreover the expression of E-cadherin was negatively correlated to that of MMP-9 ( P <0.05).Conclusion The low expression of E-cadherin and high expression of MMP-9 may be important biological indexes of invasion and metastasis of lung cancer .
