测定 CT 值变化对少量渗血出血性脑梗死的诊断价值

摘    要:目的:探讨疑似少量渗血部位与同期梗死部位CT值差值(ΔCT值)变化对少量渗血出血性脑梗死的诊断价值。方法回顾性分析2011年10月至2015年6月符合入选标准的60例患者CT、MRI图像。以MRI检查结果为“金标准”,将CT疑似少量渗血出血性脑梗死患者分为2组:A组MRI证实为少量渗血出血性脑梗死患者42例;B组MRI证实单纯缺血性脑梗死患者18例。分别测量上述2组缺血性梗死疑似渗血部位、同期梗死部位平均CT值,并计算出其ΔCT值。利用统计学软件对2组疑似渗血部位与同期梗死部位ΔCT值进行统计学分析;找出2组疑似少量渗血部位与同期梗死部位ΔCT值的最佳诊断界点。结果 A组疑似少量渗血部位、同期梗死部位CT值及其ΔCT值分别为21/.0~63.0 Hu、11.0~24.0 Hu和8.0~45.0 Hu;B组疑似渗血部位、同期梗死部位CT值及其ΔCT值分别为20.0~28.0 Hu、9.0~23.0 Hu和5.0~14.0 Hu。2组疑似渗血部位与同期梗死区ΔCT值差异有统计学意义( P<0.05)。ΔCT值为12.5 Hu是最佳诊断界点。结论当疑似渗血部位与同期梗死部位ΔCT值≥12.5 Hu时,提示缺血性脑梗死区有少量渗血可能性大;当疑似渗血部位与同期梗死部位ΔCT值<12.5 Hu时,提示缺血性梗死区内少量渗血可能性小。

The diagnostic value of CT value in a small amount errhysis hemorrhagic cerebral infarction
Abstract:Objective To explore the diagnostic value of the difference of CT value (ΔCT value ) between suspected a small amount of errhysis part and the same part of cerebral infarction in the diagnosis of a small amount errhysis hemorrhagic cerebral infarction .Methods The clinical data including CT , MRI images of 60 patients that were correspondent with the selected standard from November 2011 to June 2015 were retrospectively analyzed .Taking MRI examinaton results as “gold standard”, the patients with a small amount errhysis hemorrhagic cerebral infarction were divided into two groups :group A (n=42) in which the patients were confirmed by MRI as a small amount errhysis hemorrhagic cerebral infarction ;group B ( n=18 ) in which the patients were confirmed by MRI as simple ischemic cerebral infarction .The CT average values insuspected errhysis part of ischemic cerebral infarction and in cerebral infarction part at the same period in both groups were detected , moreover , their ΔCT values were calculated .The results were analyzed by using statistical soft ware to find the optimized diagnostic critical point .Results The CT average values in suspected errhysis part and in cerebral infarction part at the same period in group A were 21.0~63.0Hu,11.0~24.0Hu,respectively,and ΔCT value was 8.0~45.0Hu.The CT average values in suspected errhysis part and in cerebral infarction part at the same period in group B were 20.0~28.0Hu, 9.0~23.0Hu,respectively,andΔCT value was 5.0~14.0Hu.There was a significant difference in ΔCT value between two groups ( P <0.05).The ΔCT value of 12.5Hu was the optimized diagnostic critical point .Conclusion WhenΔCT value is≥12.5Hu in suspected errhysis part and in cerebral infarction part at the same period ,which suggests that there may exist a small amount of errhysis in ischemic cerebral infarction part .IfΔCT value is <12.5Hu,which indicates that the possibility of a small amount of errhysis in ischemic cerebral infarction part is less .
